"Living Healthy with Chronic Conditions" is a self-management program to help adults handle their chronic conditions, such as diabetes, arthritis, high blood pressure, obesity, chronic pain or anxiety and heart disease.
Participants will learn techniques to deal with frustration, fatigue, pain and isolation. They will also find out about appropriate exercise and medications, communication with family and health professionals, nutrition and evaluation of new treatments.

This evidence-based program created by Stanford University and Northern California Kaiser Permanente has shown a beneficial effect on physical and emotional outcomes and health-related quality of life. Participants reported decreased disability and fatigue, increased energy, lower health distress, more exercise and relaxation and fewer social role limitations.
Classes will be on April 14, 21 and 28 and May 5, 12 and 19. Adults of any age with chronic conditions are welcome, as well as caregivers.
