Returning to Activities After an Ankle Sprain: Why Most People Come Back Too Soon

Shin pain almost never comes out of nowhere. It usually follows a clear pattern that runners can recognize early if they know what to look for. Training is going well, your pace is improving, and then an ache shows up in the last mile. It fades quickly, so you ignore it. A couple of weeks later, the pain starts earlier, lasts longer, and eventually shows up when you walk. This isn&rsquo;t bad luck. It&rsquo;s your bone telling you it&rsquo;s under more stress than it can handle.
Bone gets stronger when you train, but it adapts slowly. If you increase mileage or intensity faster than your body can keep up, the bone begins to break down faster than it rebuilds. That creates a spectrum of injury: early shin splints on one end, full stress fracture on the other. Where you fall on that spectrum determines whether you need a short break or months away from running.
Early shin splints&mdash;what most runners feel first&mdash;show up as a broad ache along the inside of the shin. It may warm up during a run and return afterward. This is your first warning sign. Cutting back your running for a short time usually lets the bone recover. Physical therapy at this stage focuses on strengthening the lower leg, improving running form, checking footwear, and adjusting training so the problem doesn&rsquo;t return.
A bone stress reaction is the next step on the spectrum. Pain becomes sharper and more focused in one spot. It starts earlier in runs, lasts longer afterward, and may even show up at night. This stage often doesn&rsquo;t show up on an X&#8209;ray, which is why it&rsquo;s easy to miss. If you keep running through this level of pain, it can turn into a stress fracture. Managing it requires stopping impact exercise for a few weeks and getting a medical evaluation.
A stress fracture is when the bone finally fails. Pain is sharp, located at one specific point, and often present even when walking or resting. Some areas of the shin are considered high&#8209;risk because they don&rsquo;t heal well if ignored. These injuries require six to eight weeks with no impact and a slow, careful return to running. Full training may take several months.
Low energy intake&mdash;known as RED&#8209;S&mdash;is a major factor in bone injuries, especially in young female runners. Not eating enough for your training needs affects hormones and bone strength, raising the risk of stress fractures. Male runners can experience this too.Important warning signs include pain that moves from &ldquo;after running&rdquo; to &ldquo;during running&rdquo; to &ldquo;walking,&rdquo; pain at night, a sharp pain when doing a single&#8209;leg hop, and a very tender spot on the shin. These signs mean the bone is under significant stress and running should stop.
Physical therapy helps not only the injury but the reasons it happened: strength, mechanics, footwear, and training habits. Shin Splints to Stress Fractures: What Every Runner Needs to Know

You probably remember the exact moment it happened &mdash; a sharp pull in the back of your thigh while walking fast, swinging your leg forward, or getting up quickly. You rested, iced it, stretched gently, and after a while it felt better.
Then it happened again.
If you&rsquo;ve had more than one hamstring strain, it&rsquo;s not because you&rsquo;re unlucky or weak. Hamstring strains simply recur easily when the muscle hasn&rsquo;t fully healed or regained the right type of strength. Understanding why helps you finally break the cycle.
<h3>What a Hamstring Strain Really Is</h3>
Your hamstrings are three muscles along the back of your thigh. They bend your knee, move your leg backward, and control your leg as it swings forward when you walk. A strain occurs when the muscle fibers stretch too far or tear.
Mild strains usually heal in 1&ndash;2 weeks.
Moderate strains take 4&ndash;8 weeks.
Severe strains may need medical care.
Most people fall into the mild&#8209;to&#8209;moderate range &mdash; exactly where re&#8209;injuries are most common.
<h3>Why Hamstring Strains Keep Coming Back</h3>
1. Scar tissue wasn&rsquo;t strengthened. 
After a strain, your body repairs the area with stiff, less&#8209;stretchy scar tissue. Without rebuilding strength &mdash; especially the strength needed to control the leg as it lengthens &mdash; that spot becomes a weak link.2. Pain went away before the muscle was ready. 
You may feel fine walking or climbing stairs, yet the injured hamstring can still be 15&ndash;20% weaker than the other side.
3. Eccentric strength wasn&rsquo;t restored. 
Hamstrings must be strong while lengthening. Most home programs skip this essential step.
4. Activity resumed too quickly. 
Brisk walking, yard work, or exercise classes can overload a hamstring that hasn&rsquo;t been gradually reconditioned.
5. Hip flexor tightness or posture issues add strain. 
A forward&#8209;tilted pelvis keeps the hamstrings on stretch, making them more vulnerable.
6. The sciatic nerve may be involved. 
Nerve irritation can mimic tightness, limit healing, and make the muscle feel weak or sensitive.
<h3>What Effective Treatment Should Include</h3>
1. Gentle movement. 
Total rest slows healing. Light motion and pain&#8209;free activation help tissue heal in a more functional way.
2. Gradual strengthening. 
Progress from easy exercises to more challenging ones. The hamstring must be strong through its full range.
3. Eccentric exercises. 
These teach the muscle to control lengthening &mdash; the moment strains usually occur.
4. Strength testing before full activity. 
You should be within about 10% of the other leg before returning to higher&#8209;demand tasks.5. Address contributing factors. 
Hip flexor tightness, pelvic alignment, glute strength, nerve mobility, and walking mechanics all matter.
6. A gradual return to speed and load. 
Even non&#8209;athletes need a step&#8209;by&#8209;step return to normal walking speed, stairs, hills, and classes.
<h3>When to See a Professional</h3>
Seek an evaluation if you felt a pop, have bruising down the leg, pain near the sit bone, symptoms lasting longer than expected, or repeated strains in the same spot. A physical therapist will assess strength, flexibility, posture, nerve mobility, and movement patterns to create a plan that works for your body.

Hamstring Strains: Why They Keep Coming Back and How to Break the Cycle

I decided this week I would write a more personal note rather than send an informational blog. They seem to be read more than the information ones.
People really like to know what you are up to and thinking about and experiencing.
Usually on Mondays I go into the studio and take an intense ballet class. Today I decided to skip it.
This is a huge decision for me and one that unfortunately gets filled with some guilt.
If you are a dancer or workout regularly, you know how hard it is to take time off and give yourself some recovery time - whether active recovery or not.
With dancing in particular, it is hard on the body to take the time off, but today's decision was driven on a mental break vs. a physical one. If I can't be in dance class in my entirety (mental and physical), I know I shouldn't go.
I didn't sleep well last night knowing I had to make a decision that in the past would have been an easy one. I find it difficult to say no to helping someone, unless I know for sure that this person would not be a good fit.
I am making new decisions this year based on what my body and mind can handle, whereas previously it would have been just my mind. This is helping me to preserve myself for continuing to do what I love to do.
As I get older, I am finding I am more busy with scheduling my days between work, my passions, appointments. It almost feels like being back in the sandwich generation where you have both your family issues to take care of as well as those of your parents. Not sure how we all survive that, but we are resilient humans.
Covid was a great time for me to learn how to slow down. I truly needed that, but somehow things seem to be ramping up again even as I progress forward on projects/issues I need to deal with or have completed. There always seems to be more around the corner.
I recently went to a Graham Nash concert. Now of the 3, or 4 depending on who you think is actually part of the group, Nash was my least favorite. But I hesitated in seeing David Crosby's last concert and then he passed away.
I was told what he would be singing and decided we needed to go ( so I also don't get those regrets returning). Somehow the concert made me see the life I led and continue to lead and I have no regrets. I was grateful and thankful for what I have accomplished and what I did and continue to do in my life.
I am thankful/grateful for the health I have worked so hard to achieve, what I have done for others, and for being able to follow through with my passions, and raising my family to be independent thinkers and follow their dreams/aspirations.
So what I am trying to say as I ramble on with my thoughts, is give yourself a break, whether it be mindful or physical or both. Don't be so hard on yourself. Things usually have a way of stil resolving that suits you.
