Acton Health Department needs YOU!

The Acton Health Department is studying Substance Use Disorder and has created a Substance Abuse Disorder Coalition (SUD). Unfortunately this disorder has reached all ages and populations so in order for us to better understand the scope of this issue in Acton we need your help to gather information. This is for Acton Residents only.

The SUD coalition worked together to create this ANONYMOUS survey and these results will help us learn how widespread the problem is and what kinds of educational programs and support we should provide.
