City Of Braintree: Partnership Community Survey

The Braintree Partnership is a community prevention coalition that works to support the behavioral health needs of Braintree residents.  During the month of August, the Partnership is conducting a brief survey for residents 12 years of age and older.  The survey, which takes approximately 10 minutes to complete, will help the Partnership gain a better understanding of the resident’s needs and knowledge of available resources.  Participation is voluntary, responses are anonymous and answers cannot be traced back to those who participate.

Your candid feedback will assist us in determining how to best promote, share, and advocate for the most valued and appropriate community resources.
