A Sandwich and a Story: Understanding Spina Bifida and Arsenic in Bangladesh

How the intersection of eviornmental health and nutrition shapes global health.

A sandwich and a glass of water doesn't seem like the beginning of a global health story.


Hidden inside the meal is folate, a powerful nutrient that prevents birth defects around the world.


Scientists have known for decades that receiving enough folate is crucial in lowering the risk of a neuromuscular disorder known as spina bifida. Because of this finding, many countries (including the United States), fortify foods such as flour and bread with folic acid, the stable form of folate.

Recent research by Dr. Maitreyi Mazumdar and her team suggests that in some parts of the world, an environmental factor might interfere with folic acid's protective effects.

What is Spina Bifida?


Spina Bifida is a birth defect in which a baby's neural tube fails to close completely during the first month of pregnancy. Thus, the child's spine and spinal cord form irregularly.


The most severe form of spina bifida is called Myelomeningocele, which occurs when part of the spinal cord develops outside the spine. Children with this condition experience paralysis, difficulty walking, bladder and bowel issues, and fluid buildup around the brain (hydrocephalus).


Treatments for spina bifida have improved significantly over the years. However, the best course of action is to prevent the condition from happening in the first place.


Why Does Folate Matter?


Folate, also called vitamin B9, plays an essential role in making DNA and allowing growing cells to divide. Without it, babies suffer from severe birth defects, including neural tube defects.


Doctors recommend that women consume proper amounts of folic acid, even before pregnancy. Because neural tube development occurs so early in the process, it is vital to maintain the appropriate amount of nutrients.


Several public health programs that fortify food with folic acid have reduced the number of babies with neural tube defects.


Nutrition Isn't the Whole Story


When Dr. Mazumdar and researchers from Boston Children's Hospital were studying communities in Bangladesh, they noticed something unusual.


Children born to mothers who had healthy levels of folate still experienced higher rates of neural tube defects.
One explanation was arsenic, a natural element that was contaminating the country's groundwater and prohibiting access to clean drinking water.


The study found that folic acid offered more protection to mothers with lower arsenic exposure. On the other hand, that protective benefit was much weaker in mothers with high arsenic exposure. It seemed that arsenic itself was canceling out the effects of folate.


Scientists and researchers are continuing to investigate why this happens, but the message is clear. Improving health often requires addressing multiple factors at once, such as environmental and nutritional.
