MONDAY, JUNE 29

12:21 a.m.: Report of an erratic driver leaving the parking lot of the Lahey Hospital and Medical Center, 41 Mall Road. An officer spoke with the driver who said she was confused and suffered from post-traumatic stress disorder. She was told to go to the hospital to be evaluated, and she agreed.

2:53 p.m.: Investigation of a two-vehicle crash without injuries, Bedford Street at Church Lane.

3:40 p.m.: Investigation of a two-vehicle crash without injuries, 100 block of Middlesex Turnpike.

5:15 p.m.: Report of shoplifting, Victoria’s Secret, 75 Middlesex Turnpike. Courtney Cates, 32, of Knightdale, N.C., was arrested and charged with shoplifting $100+ by concealing merchandise.

TUESDAY, JUNE 30

10:37 a.m.: Individual reported that he believes an ex-employee had stolen his personal information and used it to assume his identity.

12:33 p.m.: Report of a past theft. About $3,000 worth of jewelry was reported stolen over the weekend, 0-100 County Road.

1:24 p.m.: Investigation of a four-car accident without injuries, Winn Street and Peach Orchard Road.

2:15 p.m.: Investigation of a two-car accident without injuries, Wilmington Road and Beaverbrook Road.

6 p.m.: Report of a dead animal, 41 Wilmington Road.

8:54 p.m.: Report of baby ducks in home pool, 0-100 block of Heritage Way.

9:06 p.m.: Report of damage to a parked vehicle, Cheesecake Factor, 75 Middlesex Turnkpike. Victim said his car was keyed while he was inside the restaurant between 8:30 and 10 p.m.
