Get Your Smart Choices Application from MGH

Massachusetts General Hospital has contributed funding for “Smart Choices for Charlestown” for 2012. The program is designed to help Charlestown organizations meet the health and human service needs of the community.

Massachusetts General Hospital’s (MGH) Community Health Associates will coordinate the application process. Funding for a one-year period will be awarded to organizations serving the Charlestown community.

Applicants or groups of applicants with innovative ideas are greatly encouragedto apply.

The deadline for submission is 12:00 Noon on Friday, May 25, 2012. Applications will not be accepted after this time. Awardees will be notified in July.

PLEASE NOTE: Applications will be distributed at a one hour information session on Monday, April 2nd, from 6:00-7:00pm. Location: Schrafft Center, 529 Main Street, 5th Floor. Refreshments will be served. Attendance at this meeting is required to receive an application. One application will be given per organization. No applications will be mailed. Please plan to be there for the entire meeting and you may not pick up anapplication for another agency.
