Hopkinton Patch has blank copies of the forms town officials use for assessment. Try them.
Here are the review forms that will be used to assess the job performances of Police Chief Richard Flannery, Fire Chief Ken Clark and Town Manager Norman Khumalo.
The final review, which will be compiled by selectmen Chairman Todd Cestari, is a public document.
The reviews will be carried out over the summer.

Here's the process:
1. The chiefs and town manager assess themselves.

2. Each selectman gets a copy of these self-assessments and, using the same format, assess the chiefs and Khumalo.
3. From this, selectmen Chairman Todd Cestari develops a final document that will be made available to the chiefs, Khumalo and the public.
The first page of the assessment has goals and asks the respondent how well the person being rated has met them according to measures of success which are part of the chiefs' and town managers' job descriptions.
The second page could roughly be described as strengths and weaknesses, though weaknesses, as in most contemporary Human Resources documents, are cast as development opportunities.
The next page has a five-category, multiple-choice list from "Clearly Outstanding" to "Unsatisfactory."
A fourth page has space for managerial and employee comments, and the fifth page is for signatures and acknowledgement of the document's content.
Copies of the job reviews are included here. How do you measure up in your job?
