My Life, My Health: The Chronic Disease Self-Management Program

Mount Auburn Hospital and Somerville-Cambridge Elder Services present “My Life, My Health,” a six week FREE workshop for adults living with, or caring for, someone with persistent health conditions including: chronic pain, limited mobility, decreased energy, frustration, and other negative impacts on your social life and daily activities due to ongoing health conditions.

The Chronic Disease Self-Management Program was developed by researchers at the Stanford University Medical Center and has been proven to help people just like you! Participants will learn a number of effective strategies on living with a health condition and will receive a copy of the book “Living a Healthy Life with Chronic Conditions” and relaxation CD.
