Hallmark Health System to offer ‘My Life, My Health’

 

Is living with a persistent health condition preventing you from really living? If so, join Hallmark Health System for “My Life, My Health,” a six session community workshop designed to give adults with any chronic condition for example, asthma, heart disease, cancer, diabetes, depression, fibromyalgia or arthritis the knowledge and skills needed to take a more active role in their healthcare.  If you are caring for someone with a chronic health condition, you are welcome to attend as well.

My Life, My Health meets on Wednesdays, Feb. 22 to March 28 from 1:30 to 3 p.m. at 41 Montvale Ave. in Stoneham.

 

This Chronic Disease Self-Management Program is an evidence-based program developed by Stanford University Medical Center and has been proven to help people just like you.
