To schedule an appointment for the Flu Shot (Ages 5 and older). IMPORTANT: click link below and scroll down and select the clinic labeled SUDBURY TOWN WIDE FLU CLINIC. Click Here for LINK

• Bring Insurance card(s). Both Medicare and supplemental insurance cards 
• Stay at home and/or cancel your appointment if you are sick.
• Bring and wear masks throughout the clinic (even if not receiving a shot).
• Bring your own snacks for recovery purposes. 
• Wear short sleeves.
• Arrive at scheduled time.
