Weston Flu Clinic: Where to Get Vaccinated

WESTON, MA – The Weston Board of Health will hold its annual flu clinic on October 25 in the Community Center Great Room.

To minimize your wait time, residents are encouraged to sign up beforehand. Follow these steps to schedule a visit:

1. Click this link to go to the invitation page on SignUp.com
2. Enter your email address – you will not need to register an account on SignUp to do so
3. Choose your time slot and confirm

After you sign up for a time, you will receive an email with a link to the required clinic forms. Please fill out one application for each person who will be getting vaccinated, and bring your insurance card and applications with you to your visit.

If you misplace the confirmation email, click here for the required forms. For those over 19 years of age, use the Private Flu Vaccine Form. For those younger than 19, fill out the State Supplied Flu Form.
