Central MA Nursing Facility Earns Top Marks In State Inspection

LANCASTER, MA — A nursing facility in Worcester County recently achieved a deficiency-free score in its most recent inspection by the Department of Public Health.

River Terrace Rehabilitation & Healthcare Center in Lancaster earned the rare distinction following an unannounced state survey that reviewed all aspects of resident care, including staffing, cleanliness, food service and residents’ rights. Only 8 percent of Massachusetts nursing homes received zero deficiencies during inspections conducted in 2025, according to the facility.

The Department of Public Health routinely conducts unannounced inspections, known as surveys, at all licensed nursing facilities across the state. The comprehensive, multi-day evaluations are designed to ensure facilities meet state and federal standards for patient care and safety.

“Achieving a deficiency-free score is a testament to the dedication and compassion of our entire team,” said Samson Girmay, administrator at River Terrace Rehabilitation & Healthcare Center. “This is a powerful example that demonstrates that when a family entrusts us with the care of someone they love, they are in great hands.”

River Terrace, located at 1675 Main St. in Lancaster, is an 82-bed skilled nursing facility that provides long-term care and subacute rehabilitation services. The center also has a five-star overall quality rating from the Centers for Medicare & Medicaid Services.

The facility offers individualized pulmonary and orthopedic rehabilitation programs for patients recovering after hospitalization.

River Terrace is also certified by the American Heart Association for heart failure care, recognizing its adherence to evidence-based standards in patient education, care coordination, clinical management and quality improvement.
