Oakland Community Health Network Releases Request for Quotation (RFQ)

OCHN is releasing Request for Quotations (RFQ) for Member Experience Survey (RFQ 0188.2023).

OCHN is soliciting quotes from qualified survey providers with expertise and experience implementing and managing the CAHPS behavioral health survey, Experience of Care and Health Outcomes (ECHO) for Adults and Children. OCHN would like the survey to take place prior to 8-30-23and be conducted according to the Instructions for Fielding the ECHO Survey 3.0 Instructions located on the CAHPS website, www.cahps.ahrq.gov.

The purpose of this RFQ is to contract with the best qualified Proposer to help OCHN examine and understand member perspectives and to continuously improve the experience of care delivered throughout our behavioral health services.

Work required under this contract and the quote that needs to be submitted by interested proposers is explained in this Request for Quotation (RFQ). Proposers should review the RFQ and submit proposals in compliance with the RFP. The RFQ is designed to provide Proposers with all the information needed to write a successful quote. Required Attachments are provided in Word format for simplicity in preparing a quote.

Proposal Requirements

1. Proposer must be NCAQ certified.

2. Proposer acknowledges and understands that any data collected while performing required work is not permitted to be used or disclosed for any other purpose outside of the scope of work for this contract.

3. Proposer acknowledges and understands the contract resulting from this solicitation will require the successful Proposer to sign a Business Associate Agreement (BAA) with OCHN.

4. Proposer acknowledges and understands that any changes to an OCHN contract scope of work must be made through a formal change management process.

5. Proposer must designate one contact person who will be responsible for the solicitation process and any resulting contract with OCHN.

6. Neither the Proposer, nor any of its employees or contractors engaged to provide goods and services to OCHN shall be excluded from participation in Medicare, Medicaid, or any other Federal programs.

7. Proposer must meet the insurance requirements outlined in Attachment F Insurance and supported by Attachment G OCHN Professional Services Contract.

8. Will you be using subcontractors to accomplish the OCHN scope of work?

To be eligible to submit a quotation, a completed Request for Quotation shall be submitted electronically to Jen Hillier, at hillierj@oaklandchn.org by 3:00 p.m. EST, December 5th, 2022.
