Understanding the Person Centered Plan

Learn what you can do to improve the implementation of a person's services and have input that may improve a person's plan.

Participants Will:
 Learn what you can do to improve the implementation of a person’s services and
have input that may improve a person’s plan.
 Better understand the importance of the guidelines and standards of care.
 Learn what options and resources are available to you to help you provide better
services and supports to those in your care.
 Learn how to be a better advocate for those you serve by asking for clarification and
instructions on the IPOS/PCP.

Persons Who Should Attend:
 Direct care workers
 Personal care assistants
 Caregivers
 Family members
 Anyone working in self-determination settings providing care
