OCHN Health Home model adds substance use disorder services

The community mental health agency is now offering a Substance Use Disorder (SUD) Health Home service model.

OAKLAND COUNTY, MI (April 26, 2022) – Oakland Community Health Network (OCHN), along with its provider network, launched the Behavioral Health Home (BHH) initiative in late fall of 2020. Two years later, the community mental health agency is now offering a Substance Use Disorder (SUD) Health Home service model.

“We are very excited to provide this unique and effective service model to support recovery for individuals with substance use disorders in Oakland County,” states OCHN Executive Director and CEO, Dana Lasenby. “The Substance Use Disorder Health Home model promotes access to community supports and services so individuals can improve their whole health outcomes.”

A Substance Use Disorder Health Home is a personalized model of care that coordinates physical health, behavioral health, and social needs to produce long-term benefits. The service benefits Oakland County residents who have a physical health condition and SUD diagnosis. Health Home participation expands traditional SUD services to include emphasis on coordinating care with a person’s medical practitioner and other community supports and resources.

Easterseals Michigan, Oakland Family Services, and Sacred Heart are the current OCHN network providers participating in the SUD Health Home program.

About OCHN

OCHN is contracted by the Michigan Department of Health and Human Services to lead a provider service network that assists approximately 23,000 Oakland County citizens at more than 300 service sites across the county. People who receive public behavioral health services through OCHN's provider network include those who have an intellectual or developmental disability, mental health challenge, or substance use disorder. Most of these individuals have Medicaid insurance coverage.

OCHN's goal is to ensure these individuals are aware of and have access to services and supports that will improve their health and quality of life, as well as ensure their engagement in full community participation. Its mission to “inspire hope, empower people, and strengthen communities” reflects an unyielding belief in a "Valuable System for Valued People."
