• Stillwater Mine electrician who died in 2025 used wrong switch, report shows (ktvq.com) — A federal investigation has concluded that last summer’s fatal electrocution at the Stillwater Mine near Columbus stemmed from an unlabeled power switch and inadequate training on a new shutdown procedure. Regulators cited the mine for multiple safety failures and say the company has since updated policies, training, and labeling practices. The report also notes three other worker deaths at the mine since 2021.
