HealthFirst Earns Highest Level Patient-Centered Medical Home Status Once Again

Franklin, NH: HealthFirst Family Care Center announced today that both their Franklin and Laconia sites have once again earned Level III Patient-Centered Medical Home (PCMH) recognition by the National Committee for Quality Assurance (NCQA). Research demonstrates that the PCMH model leads to higher quality of healthcare at lower costs, while increasing patients’ and providers’ experience of care and satisfaction.

To achieve the highest level of PCMH, healthcare facilities must prove they know their clients well, monitor health conditions, and deliver outstanding results.
