Mexico City has less than 50 licensed municipal ambulances despite a population of nearly 9 million, according to the film. The family acts as a crucial, but unregistered, underground lifeline in an occupation riddled with police bribes and cutthroat competition.

The family has a reputation for dependability and trustworthiness but becomes faced with sudden escalation in demanded bribes. This could force them into ethically questionable practices to keep their dire financial situation from compromising the patients in their care.
