CentraState Healthcare System is now offering the free program, “Take Control of Your Health: A Chronic Disease Self-Management Course,” to help people manage chronic conditions, feel better, gain control, and do the things they want to do.
The free, 6-session workshop is on consecutive Thursdays beginning Aug. 8, 1:30-3:30 p.m. at CentraState’s Star and Barry Tobias Health Awareness Center, 901 W. Main Street, Freehold.
The course—which was developed at Stanford University—is designed for people age 55 or older who have chronic health conditions, including but not limited to diabetes, high blood pressure, arthritis, or persistent pain. Participants will get the support they need to take control of their health, find practical ways to deal with pain and fatigue, discover better nutrition and exercise choices, understand new treatment choices, and learn better ways to talk with their doctor and family about their health.
