To participate in the program please bring:
- Immunization record for child
- Parent/Guardian photo ID
- Child's birth certificate
- Medicaid/Insurance card
- Proof of current Newark residency*
*Utility, telephone or cable bill. All documentation must be current, within the last 30 days of the time of visit.
