Spring Grove Rehab & Healthcare Introduces Chronic Illness Support Model in New Providence, N.J.
Skilled Nursing Center Partnering with HealthDrive

Spring Grove Rehabilitation & Healthcare Center in New Providence has launched a comprehensive Chronic Illness Support model, partnering with HealthDrive to integrate palliative care, pastoral support and other services with its specialized clinical programming. The 106-bed skilled nursing facility, located at 144 Gales Drive, provides post-hospital care, short-term rehab and long-term residential care.
The patient-centric Chronic Illness Support model is available to clients of Marquis Health Consulting Services, which supports the facility. “Spring Grove’s new program is truly a next-level model of care, and it is strengthened by an excellent partner in HealthDrive,” said Marquis’ Christina Albano, regional director of market development. “There is great value in providing a holistic focus on both physical and spiritual wellness for patients.”
The palliative care component of Spring Grove’s Chronic Illness Support program is administered by a trained team of doctors, nurses and palliative specialists. The initiative focuses on providing expert medical care that includes emotional and spiritual support to manage chronic conditions and help relieve symptoms and stress. The goal is to enhance quality of life for patients and their loved ones.

Subhra Sen, APN, is leading the Spring Grove program for HealthDrive. She is a board-certified adult-gerontology and primary care nurse practitioner with extensive in-patient medical experience and advanced training.
The new program at Spring Grove is specifically supporting the facility’s specialty initiatives, including pulmonary care and cardiovascular medicine. It also weaves in disease-specific education and health coaching, multi-specialist led and coordinated care, social services support, and connections to community resources and life enrichment activities.
