Human Services Needs Assessments Form

The attached 2020 Needs Assessments form for Paramus residents can now be completed online. If you need help accessing care or resources, please complete the form and email it to: imaury@paramusborough.org.

The form is for anyone in need of help. You do not need to complete the financial section if that is not your concern.

If you are interested in using the Paramus Community Food Pantry, you will need to complete the financial section. Human Services is free and confidential for Paramus residents only.
