Easterseals New Jersey announces that outpatient behavioral health services previously provided through CenterPath Wellness at the Plainfield, New Jersey location have concluded effective January 15, 2026.



Individuals who received services were offered assistance with care coordination, referrals to local community providers, and the opportunity to continue services at Easterseals New Jersey’s outpatient office located at:

Former service recipients may request copies of their medical records or authorize transfer of records to another provider by contacting the office listed above. Medical records will be maintained in accordance with all applicable state and federal regulations.
