How Healthy Are You, Chatham, Summit And Westfield?

SUMMIT, NJ - Residents of Chatham Borough, Fanwood, Garwood, Mountainside, New Providence, Roselle Park, Summit and Westfield are being asked to participate in a new survey designed to help try to identify which topics should be focused on to improve public health within the rea.

This survey is the chance for residents to decide what should be focused on. It should take about 15 minutes and questions can be skipped. The answers are completely confidential. Participants who would like to get a copy of the results for their community can add your contact information at the end.
