Hackensack Meridian Health (HMH) Collaborates with Alameda Center for Rehabilitation and Healthcare
Alameda Center selected as one of its preferred Sub Acute Rehabilitation (SAR) provider for The Comprehensive Care for Joint Replacement

Hackensack Meridian Health (HMH) at Raritan Bay Medical Center (RBMC) recently announced it has selected Alameda Center for Rehabilitation and Healthcare as one of its preferred Sub Acute Rehabilitation (SAR) providers for hip and knee Total Joint Replacement (TJR). Medicare patients undergoing total knee or hip replacement at RBMC will be offered Alameda Center for rehabilitation if SAR is deemed necessary by the orthopedic surgeon.
Under the Affordable Care Act, Medicare is launching several bundled payment initiatives in order to improve quality and patient outcomes. Comprehensive Care For Joint Replacement (CJR) is the first bundle payment initiative launched in the area. Under the bundle CJR initiatives, hospitals usually align themselves with vibrant, well-positioned facilities with three or more Medicare Star rating to care for the patients. In the case of Alameda, it is the only Four Star Medicare rated facility in the area.
