City School District Of Albany: COVID-19 Student Vaccination Survey

The City School District of Albany is planning for school-based COVID-19 vaccination clinics for students in partnership with the Albany County Health Department and Whitney Young Health Services.

To help our partners plan for the best locations and the number of sites needed, we are asking families of children age 5 and older to complete a short survey. The survey will take about a minute. There is no login required.

We will provide more information about vaccination clinics after collecting your feedback. 

We appreciate your input as we work together to slow the spread of this disease in our community.
