COVID-19 Rapid Test Kit Request Form

New York State has provided the Central Islip School District with COVID-19 Rapid Test Kits to distribute to students to bring home so that they can test twice in the week after they return from the winter break.

If you wish to receive a COVID-19 Rapid Test Kit for your child/children, please complete the request form below. You will need to complete one form for each student. Requests will be filled by the end of each school day February 14, 2022 through February 18, 2022, and will be sent home with your child. Please note, requests received by 10 a.m. will be filled that day; requests received after 10 a.m. will be filled the next school day. If your child is absent from school or you prefer to pick up a test kit, please contact your child's building to arrange a pick up time. COVID-19 Rapid Test Kits will be given out on a first-come, first-served basis while supplies last.
