Free Flu Shot Immunization
For Seniors 60+ Thursday, October 23, 2014 from 10:00 a.m. to 1:00 p.m. in the Levittown Public Library Community Room
This program is sponsored by NuHealth, with coordination of the library. For Seniors 60+, spouse, and caregiver of any age.
Please arrive 15 minutes prior to your scheduled time to complete the required forms. There is no out of pocket expense for the immunization for any senior citizen, 60 years of age and older, and their spouse, or care giver of any age. Please bring proper identification, your Medicare card, or other primary insurance card. It is not necessary to have insurance to receive a free flu shot. The pneumonia shot will also be available. Shots will be given to anyone not pre-registered or who miss an appointment time based on availability of vaccine and available time slots. Pre-registered patrons will be taken first.
In-person or by phone registration begins for Levittown School District Residents on Thursday September 18, and non-residents on September 25, at the Reference Desk. Call the library at 516-731-5728 to register.
