Sonia Flores Wins Planetree Award

HRHCare Community Health is pleased to announce that Sonia Flores, a patient representative at HRHCare Greenport, has been awarded the May 2017 Planetree Spirit Award for Excellence in Patient Care. The HRHCare Planetree Institute presents these awards to exemplary employees who work to make health care more personalized, and create a warm and welcoming environment for all.

As a patient representative, Sonia Flores is the first face that patients see as they walk into the HRHCare Community Health Center in Greenport. A part of the HRHCare family for the past five years, Sonia is well known in the health center, as well as in the Greenport community at large, for going above and beyond to make sure that every patient has access to the care that he or she needs. Her smile and kind words have touched countless HRHCare Greenport patients, and she always takes the time to answer questions on everything from how to make an appointment to helping the patient understand the advice that their provider has given. Sonia is also known as the “ACO Champion” for Greenport, doing whatever she can to ensure that the health center meets its requirements in tying payment to quality metrics and the cost of care as an accountable care organization.

“Sonia understand the true importance of diversity, and I could not be prouder to have her on our team,” said Cassaundra Mariotti, HRHCare Suffolk Operations Manager. “She truly deserves this award for her dedication to the community. We are so fortunate to have her as part of our team.”

Sonia is passionate about the mission of HRHCare to provide access to affordable care for everyone. When asked what she thought patients most need from their provider, she replied, “Empathy, flexibility, and good communication.” She also participates in many community outreach activities, including the local strawberry festival and the HRHCare “Days of Health” which provide health care services to Suffolk County agricultural workers and their families. In her spare time, Sonia enjoys spending time with family, traveling, canoeing, playing beach volleyball, singing, dancing, and exploring different cultures and types of food.
