Options for Community Living Launches New Care Coordination Services for Long Island's Children
Options for Community Living, Inc. is pleased to announce that it now provides comprehensive care management for Medicaid -eligible children on Long Island with two or more chronic health conditions, serious mental illness and/or HIV/AIDS. “Like our program serving adults, our care coordinators will work in cooperation with area Health Homes to best meet the needs of children,” stated Yolanda Robano-Gross, executive director.
A health home (not a physical location) is a network of providers who work together to ensure that an individual’s health care needs are being met. Health home providers operate under a "whole-person" philosophy and will integrate and coordinate primary, acute, behavioral health, and long-term services and supports to treat the whole person.
An Options care coordinator will help develop a care plan for a child that details specific needs and goals as well as the various providers and services that are required to achieve those goals. Care coordinators help arrange doctor visits, pharmacy accommodations, mental health treatment, substance use disorder services, early intervention, specialty services and more.
