LMSD is asking parents to fill out their Senior Name Form for the 2020-2021 yearbook by Friday, Nov. 6, 2020.

Please complete the Senior Name Form and return to Mrs. Queroli by Friday, November 6, 2020 to indicate the name of your student as it should appear on their diploma and in the yearbook. If this form is not received by Friday, November 6, 2020, your student's name will appear on their diploma and in the yearbook as first and last name only. Names cannot be changed once the diploma and yearbook are printed. This form may be returned in the "Senior Name Form" box in the Main Office OR by email.
