The facility is located on 1180 Welsh Road, immediately on line between Upper Gwynedd and Montgomery townships.
The Plaza was developed following a multi-year Asian American and Pacific Islander Community Strengths and Needs Assessment study. The analysis found gaps in care for that community in Montgomery County and the surrounding area, often caused by struggles with language services and related support systems.
