SET-UP:  7:00am

AT THE FRIENDS STATION:  Complete the attached VENDOR REGISTRATION FORM with all pertinent information.  SIGN & DATE the form.  Detach it from this cover sheet and place the form and a CHECK payable to FRIENDS OF HV LIBRARY in the envelope provided.  Drop the envelope into the white box on the wall.
