A nationally recognized Provider of All-Inclusive Care for the Elderly, Mercy LIFE (Living Independently for Elders) is now available to Delaware County seniors. This expansion means area residents now have access to comprehensive and compassionate medical care (without co-pay charges) and nutritional, social and spiritual resources through Mercy LIFE that will enable them to live independently in their own homes or with family.

Mercy LIFE offers specialized home care to participants who need assistance getting ready in the morning and assistance at night preparing for bedtime – an invaluable service for busy working caregivers who can’t always count on being home at night, or for seniors who live alone. Transportation to and from the Mercy LIFE Day Centers – where participants receive their healthcare services, socialize and participate in recreational activities – is also provided.
In addition to offering services to participants in their homes, seniors enrolled in the Mercy LIFE program gain access to interdisciplinary care teams who provide preventive healthcare, and observe and respond to changes in each participant’s health. Nurses, physicians, physical therapists, nutritionists and other healthcare providers work on-site to meet seniors’ healthcare needs swiftly and efficiently. Mercy LIFE participants benefit from immediate assistance – without the typical wait period for a medical appointment or prescription refill, or the costly co-pays that often accompany the following vital services:

Mercy LIFE participants receive a high-level of quality care: the program’s participant-centered care model has reduced hospital readmission rates to below 11.1 percent in the first quarter of 2013, well below the national rolling average of 20 percent.
