Mercy LIFE provides complete medical care for seniors, working as a team to help participants keep illness under control, live independently and prevent hospital visits. Recently, Mercy LIFE became available to seniors living in Delaware County, including Darby, Chester, Yeadon, Prospect Park, Wallingford, Woodlyn, Lansdowne, Sharon Hill, Garnet Valley, and Ridley Park (specific zip codes include 19013, 19014, 19015, 19018, 19022, 19023, 19032, 19033, 19036, 19050, 19060, 19061, 19074, 19076, 19078, 19081, 19079, and 19094).
At the three Mercy LIFE Adult Day Centers, participants see their doctor, nurses, therapists and social workers. They can make friends and enjoy activities and hot meals. Mercy LIFE offers transportation to and from the Adult Day Centers, to other medical appointments, and the staff makes home visits, too.

Mercy LIFE physician David Mingle, MD, believes the model helps everyone. “As a Mercy LIFE doctor, I have the support I need to keep my patients healthy. We can set up visits from a nurse, physical therapist or a home health aide.” Mercy LIFE also arranges for medications to be administered at the Adult Day Centers and at home.

A Mercy LIFE care plan worked very well for Maryanne Triadafillopoulus. She suffers from COPD and was in the hospital so often that the emergency room staff knew her well. Maryanne was referred to the Mercy LIFE program and now she’s monitored regularly. “We work hard to stop the cycle of frequent hospital visits,” says Dr. Mingle, also Maryanne’s doctor. “We are their partners in health.”

After joining Mercy LIFE, Maryanne began to enjoy social visits to the Adult Day Center, where she also received counseling for stress and sadness, common side effects for seniors living with illnesses. “The Mercy LIFE staff really does care about you,” says Maryanne. “They deliver my medication to my home every day. I couldn’t do it without them.”
