Your Life, Your Choice! Advance Care Planning

Phoenixville Public Library will host a free presentation, “Your Life, Your Choice! Advance Care Planning”, on Monday, August 17, 6:30 PM, at the Library. Jennifer Lipski, RN, MA, Community Educator with VITAS Healthcare Philadelphia, will be the speaker. Join in a one-hour, RN-led conversation on advance care planning: how to make your own decisions about your future health care and ways to talk with your family, friends, and physicians about health care choices for yourself and your loved ones. Sample advance directive forms will be available for you to take home. This event is free and open to the public. The program will also be livestreamed on Zoom for those unable to attend in person.
