The data breach occurred in February, the health care conglomerate said.

PITTSBURGH, PA — A Tennessee billing contractor is contacting more than 25,000 UPMC patients after a data breach that may have left their personal information exposed.
While the nature of any affected information will vary from individual to individual, the types of personal information exposed include names, addresses, medical billing and insurance information, certain medical information, and demographic information such as date of birth and social security numbers, UPMC said in a statement posted on its website.
Intellihartx, which UPMC uses for billing and collection, is mailing notice of the incident to those potentially affected individuals for whom it has contact information. The company is offering free credit monitoring and identity restoration services to anyone whose records have been used illegally.
