Fake 'Face Mask Exemption' Form Circulates On Social Media

PROVIDENCE, RI — A fake form to apply for a so-called "medical/religious facemask exemption certificate" has been circulating on social media, the Rhode Island Department of Health said.

While the form has the department's logo, it is not an official document, RIDOH said. Neither the department or any other official source in Rhode Island has created such a form.

"This form was not developed by RIDOH or any official source, should not be filled out, and should not and will not be accepted by any entity, as the form is not a legitimate document," the department said.
