Fatal On I-295: One Dead & Two Hospitalized After Accident

State police are investigating an accident Thursday morning on I-295. A tow truck, a box truck and a Mazda were involved.

JOHNSTON, RI — One person has died as a result of a three-vehicle crash Thursday morning on I-295 near Exit 7. State police said the preliminary investigation shows a Mazda sedan, a tow truck and a two-axle box truck were traveling in consecutive order in the center lane of Route 295 North.

"At a point, approximately one-half mile North of Route 6 in Johnston, the three vehicles made contact in a rear end, chain reaction like fashion. The events which led up to the collision remain under investigation," state police said.

The driver of the box truck died as a result of his injuries. He was pronounced dead at Rhode Island Hospital. His name is being withheld pending notification to family.

His passenger, also not identified, suffered life-threatening injuries, police said. He is being treated at Rhode Island Hospital.

The operator of the tow truck, identified as Trevor Armstrong, 28, was transported by rescue to Rhode Island Hospital with non-life threatening injuries.

The operator of the Mazda was identified as Mitchell Savard, 41. He did not go to the hospital.

The State Police Accident Reconstruction Unit, Forensic Services Unit, Johnston Police and Johnston Fire and Rescue responded around 9:40 a.m. Members of the Pascoag Rescue also assisted.

The accident remains under investigation. Witnesses are asked to contact State Police Headquarters at (401) 444-1000.
