Community Care Alliance Receives $70,000 from Rhode Island Foundation to Expand Services at Landmark Medical Center
WOONSOCKET, RI – Community Care Alliance has received a $70,000 grant from the Rhode Island Foundation to expand its psychiatric hospital diversion services at Landmark Medical Center. The one-year Strategy Grant will both improve trauma-informed, wraparound care for program clients and their families, and help reduce inappropriate hospital admissions at Landmark Medical Center.
The program is slated to begin early in 2015. The target population is individuals who are in crisis and need stabilization, but not in acute need of psychiatric hospitalization. Services provided by Community Care Alliance include clinical and basic needs assessments on-site at the hospital emergency department, and follow up services for mental health and addictions treatment, and basic needs assistance.
Community Care Alliance is a multi-service non-profit health and human service agency whose mission is to support individuals and families in their efforts to meet economic, social and emotional challenges and enhance their well-being. For more information, visit www.CommunityCareRI.org.
