Arlington County Public Health officials recorded violations at several food establishments during inspections between Jan. 22 and Jan. 29.
ARLINGTON, VA — Several facilities in Arlington recorded violations during recent inspections from the Arlington County Public Health department. Inspectors conducted the most recent visits between Jan. 22 and Jan. 29
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the food service presents a substantial and imminent threat to the public health."

- Observed staff member enter kitchen and begin to prep foods without washing hands. Corrective Actions: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.
- Observed raw pork stored over cooked pork in prep table cooler, and observed that portioned frozen foods are not stored according to cooking temperature, with chicken stored over beef in freezer. Corrective Actions: Separate types of raw animal foods from each other such as beef, fish, lamb pork, and poultry during storage, preparation, holding, and display by the following: (a) Using separate equipment for each type. (b) Arranging each type of food in equipment so that cross contamination of one type with another is prevented. (c) Preparing each type of food at different times or in separate areas.
- Observed staff member washing dishes at 3 compartment sink with only soap. Sanitize basin was not filled. Corrective Actions: After cleaning and rinsing of the food-contact surface, the surface shall be effectively sanitized before coming in contact with food and before use. Instructed staff to set up the 3 compartment sink and sanitize dishes.
- Observed that tofu and wide noodles were stored on the shelf at room temperature. Corrective Actions: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below.
- Food storage containers or working containers of food ingredients that have been removed from their original containers are not labeled to identify the contents. Corrective Actions: Label working containers with the common name of its contents.

- Observed that foods were stored uncovered in reach in cooler. Corrective Actions: Protect food from cross contamination by storing food in packages, covered containers, or wrappings.
- Observed that in use dishmachine was not dispensing sanitizer (0ppm). Observed that sanitizer container was empty. Corrective Actions: Replace sanitizer so that dishes are sanitized before use and after cleaning. Dishwasher should test the dishmachine often to ensure that it is working properly. Sanitizer was replaced and tested at 50ppm
- Observed that containers of salt, flours etc were not labeled with the name. Corrective Actions: Label working containers with the common name of its contents
- Observed dead pest was found in the control device. Corrective Actions: Remove dead or trapped birds, insects, rodents, and other pests from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
- Observed that walls in the storage area in the neighboring building are in disrepair, and have holes in walls and ceilings. Corrective Actions: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities

- Back hand sink wasn't supplied with paper towels; corrected. Corrective Actions: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials
- Three part sink not set-up to wash, rinse & sanitize food contact equipment/utensils. Blender observed not kept clean. Corrective Actions: Clean and sanitize these surfaces for food contact.
- The mall food court back hallway doors leading to the loading docks aren't kept closed, self-closing, nor tight fitting. Corrective Actions: Protect the food establishment against the entry of insects and rodents by: 1. Filling or closing holes and other gaps along floors, walls, and ceilings; 2. Closed, tight fitting windows; and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
- There are mouse droppings on the floor in the storage room. There are roach droppings and dead roaches observed on kitchen walls and inside the ceiling light fixtures. Corrective Actions: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.
- Scoop/spoon found in standing water. Containers with no handles found in sugar & rice. Corrective Actions: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F and the container.
- Blenders observed not stored to air dry. Corrective Actions: Store blenders in a self-draining position that allows air-drying.
- Soda crates used as shelves instead of shelve with 6 inch legs or wheels to keep floors clean. Corrective Actions: Discard and replace these soda crates with shelves with minimum 6 inch legs or castors.
- Mall trash/recycling dumpsters aren't kept covered. Corrective Actions: Cover all waste containers when not in continuous use.
- Tiny holes in the kitchen walls. Rodent hole in the storage room wall. Mall back hallways have damaged floor & base tiles. The floors aren't kept clean. Corrective Actions: Maintain physical facilities clean and in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
- Mop not hung to air dry. Corrective Actions: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

- Observed that several foods stored in bottom of prep table coolers, foods and spices stored on the prep line, foods stored in the walk in cooler, have not been wrapped or covered to prevent contamination.
- Corrective Actions: Protect food from cross contamination by storing food in packages, covered containers, or wrappings.
- Observed a container of blanched chicken was placed on prep table top, was holding at 78F. PIC stated that it was prepared around 12:00pm Corrective Actions: Cool time/temperature control for safety (TCS) food(s) from 135°Fto 41°F within a total of 6 hours and from 135°F to 70°F within 2 hours. A longer time for cooling allows an ideal situation for bacterial growth. This has been shown to be the major contributing factor in many foodborne illnesses. Chicken was reheated by boiling, and cooling process was started again.
- Methods are not being used to control pests. Corrective Actions: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required. Contact your pest control provider to service the sushi bar and kitchen.
- Observed that the kitchen dishmachine is unable to dispense sanitizer. Corrective Actions: Contact company to service the machine, so that sanitizer is able to be dispensed per manufacturer requirements.
- Interior of establishment is in need of a deep cleaning underneath cookline equipment, under the 3 compartment sink in the sushi bar etc. Corrective Actions: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
- Facility is storing unnecessary items to the operation or maintenance of the establishment. Corrective Actions: Remove all unnecessary items from the facility. The presence of unnecessary articles, including equipment which is no longer used, makes regular and effective cleaning more difficult and less likely. It can also provide harborage for insects and rodents.

Corrective Actions: The permit holder shall designate a person in charge who is authorized to monitor and manage all food establishment operations and who is authorized to take actions to ensure that the Regulations' objectives are fulfilled and shall be present at the food establishment during all hours of operation.
There is no certified food protection manager on-site; required during al hours of operation. Corrective Actions: A certified food protection manager is required that has supervisory and management responsibility and the authority to direct and control food preparation and service and who has shown proficiency of required information through passing a test that is part of an accredited program.
The three part sink not set-up to wash, rinse & sanitize; left/sanitize basin stopper doesn't keep water inside the sink (left my QT-49 test papers with the facility, since I couldn't find any). Corrective Actions: Clean food-contact surfaces no less than every 4 hours to prevent the growth microorganisms on those surfaces.
Individual packages of blue cheese dressing found at room temperature even through label states, "Keep Refrigerated". Corrective Actions: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.
Men's rest-room toilet and kitchen hand sinks aren't kept clean. Corrective Actions: Maintain non-food-contact surfaces of equipment clean.
Mop sink back flow preventer device, 3-part sink faucet handle/right basin/drain pipe/left basin stopper and back hand sink hot water handle are in poor repair. Corrective Actions: Plumbing systems and components shall be maintained in good repair.
Shared trash/recycling container doors aren't kept closed. Corrective Actions: Waste handling containers are to be insect and rodent proof.
Facility kitchen floors along edges aren't kept clean. Corrective Actions: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. - Observed that the caulking on the sinks in both restrooms are splitting. Corrective Actions: Reseal the sink to the wall so that water cannot splash behind the sink.

The two prep units on the grill side of the cooks line were observed not cold holding properly to include cooked onions (51F), shredded cheese (47F) and sausage (50F). Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
- Reduced oxygen packages of salmon were found in the #4 refrigerator; corrected on-site by opening packages. Corrective Actions: Remove fish that has been packaged in a reduced oxygen package from the reduced oxygen environment prior to thawing in a refrigerator that maintains food temperature at 41°F or less, or immediatley upon completion of thawing if thawed by completely submerging the package(s) under running water at a temperature of 70°F for a time period that doesn't allow the thawed portions of the fish to exceed 41°F for more that 4 hours including the time to cool back down to 41°F in the refrigerator if not immediately cooked.
- The three part sink chemical dispenser back flow prevention device and the dish machine area's garbage disposal back flow prevention device aren't in good repair. Corrective Actions: Repair and maintain all plumbing components and fixtures.
- The floors under the cooking equipment and along some edges throughout aren't kept clean. Corrective Actions: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. - The food establishment is not under the control of a Certified Food Protection manager. The CFPM walked in more towards the end of the inspection. - No cold water at prep area/warewashing area hand sink. Corrective Actions: Repair the cold water at the above-mentioned hand sink and ensure to have both hot- and cold water supply for employees to effectively wash their hands as and when needed.
- Observed raw-shelled eggs stored over containers of half-&-half in "Dukers" 2-dr cookline prep ref., trays of raw-shelledggs over several RTE-foods in "True" 2-dr RIC and raw chicken over an uncovered pan of yellow rice in upstairs WIC. Corrective Actions: Separate raw animal foods from ready-to-eat foods. Raw-shelled eggs and raw chicken and other raw animal products may not be stored over ready-to-eat foods.
- When tested, no sanitizer was observed in 3-compartment sink. Corrective Actions: Provide chlorine at proper concentration of 50-100 ppm and immerse or expose food contact surfaces to sanitizing solution for adequate time.
