Arlington County Public Health officials recorded violations at several food establishments during inspections between Jan. 30 and Feb. 6.
ARLINGTON, VA — Several facilities in Arlington recorded violations during recent inspections from the Arlington County Public Health department. Inspectors conducted the most recent visits between Jan. 30 and Feb. 6.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions.
They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the food service presents a substantial and imminent threat to the public health."

- The bar dish machine was not ejecting 50-150 ppm of chlorine; corrected on-site by priming the sanitizer line, however, the tubing isn't clean in color to observe the sanitizer going through. Kitchen dish machine utensil surface temperature only reached 158.7 F instead of minimum 160 F. Corrective Actions: The white tube of the bar dish machine needs to be exchanged with a clear tube and it needs to be tested each run to ensure it is sanitizing at 50-10 ppm of chlorine. Repair the kitchen dish machine unit so that achieving a utensil surface temperature of 160°F can be measured by an irreversible registering temperature indicator (e.g. thermolabel and maximum registering thermometer). Discontinue use of mechanical ware-washing machine and convert to use of 3-vat sink wash, rinse and chemical sanitization. Use single-service items as deemed necessary if not capable of effective manual washing, rinsing, and sanitization of food-contact surfaces.
- Fruit fly/small flies present at bar. Corrective Actions: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.
- Kitchen dish machine water pressure gauge is still stuck at 15 psi and its BFP device pipe is dripping water. The walk-in refrigerator at 43 F, shall be at or below 41 F. Corrective Actions: Repair, replace, and/or maintain the calibration of gauges for accuracy and function in the intended range of use.
- Walk-in refrigerator fan covers are dusty. Corrective Actions: Maintain non-food-contact surfaces of equipment clean.
- Outside recycling dumpster lids not kept closed. Corrective Actions: Cover all waste containers when not in continuous use.
- Mop not hung to dry. Corrective Actions: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

Corrective Actions: A certified food protection manager is required that has supervisory and management responsibility and the authority to direct and control food preparation and service and who has shown proficiency of required information through passing a test that is part of an accredited program.
- The mall loading dock doors aren't kept closed, tight-fitting, or self-closing. Corrective Actions: Protect the food establishment against the entry of insects and rodents by: 1. Filling or closing holes and other gaps along floors, walls, and ceilings; 2. Closed, tight fitting windows; and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
- Fruit fly observed by mop sink. Corrective Actions: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.
- Soda crates used as shelving. Corrective Actions: Discard and replace the soda crates used as shelving to provide proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter.
- Mall trash dumpster has no lid & grease dumpster lid is not kept closed. Corrective Actions: Cover all waste containers when not in continuous use.
- A couple damaged baseboard tiles by the mop sink. Ceiling tiles dusty by vent. Corrective Actions: Maintain physical facilities clean and in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
- A few ceiling light bulbs aren't working. Corrective Actions: Provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor

- Facility is operating without a current Food Service Facility License; obtain immediately. Corrective Actions: Submit 2024 application.
- Back hand sink is out of soap; refill dispenser. Corrective Actions: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
- Three part sink not set-up to sanitizer; corrected on-site; bowls being washed and not sanitized. Corrective Actions: Clean and sanitize these surfaces for food contact.
- Eggplant dip, chickpea salad and hummus cold holding at front counter at 45-50 F; completely surround pans with ice and water to keep at or below 41 F.
- Corrective Actions: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.

- Rice found cooling with plastic wrap-->loosely or uncover until at or below 41 F; ensure cooling 135-->70 F within 2 hours & 70-->41 F within 4 hours. Corrective Actions: Arrange the food containers in the refrigeration equipment to provide maximum heat transfer through the container walls. Loosely cover or uncover the food if protected from overhead contamination during the cooling period to facilitate heat transfer from the surface of the food.
- Packaged foods in the "dessert" display case refrigerated aren't labeled with food name ingredients and allergens. Corrective Actions: Label working containers with the common name of its contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.
- Building shared recycling dumpster outside is missing its drain plug. Corrective Actions: Have drain plug replaced and keep doors closed. Waste handling containers are to be insect and rodent proof.

Corrective Actions: A person in charge shall be on duty during all hours of operation who oversees the responsibilities of the food code. Conspicuously post the foodservice facility license for the public.
- There is no certified food protection manager on-site; email/text me information. Corrective Actions: A certified food protection manager is required that has supervisory and management responsibility and the authority to direct and control food preparation and service and who has shown proficiency of required information through passing a test that is part of an accredited program.
- Hand sink by three compartment sink had items stored inside the sink and no paper towels in the wall dispenser; items were removed from sink, paper towels found on nearby shelving. Corrective Actions: Access to the handwashing facility identified above is to be available during all hours of operation. Don't store items inside this sink, preventing its use. Keep paper towels in the dispenser at this sink.
- Sanitizer wasn't set-up at 3 compartment sink. Corrective Actions: After cleaning and rinsing of the food-contact surface, the surface shall be effectively sanitized with 200 ppm of quaternary ammonium at 74 F before coming in contact with food and before use.
- No visible thermometers were observed in the prep refrigerators. Corrective Actions: Provide an easily readable thermometer in each refrigerator so employees can accurately read the thermometer.
- Two dead American cockroaches were observed on the floor of the dining room closet. Corrective Actions: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.
- Air gap at the bottom middle of entrance doors. Corrective Actions: Protect the food establishment against the entry of insects and rodents by: 1. Filling or closing holes and other gaps along floors, walls, and ceilings; 2. Closed, tight fitting windows; and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
- Containers not being air dried; observed stacked while still wet. Corrective Actions: Store containers in a self-draining position that allows air-drying.
- Right prep refrigerator condenser pan is damaged; water leaking into unit.
- Corrective Actions: Repair the refrigerator to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the refrigerator, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
- Test papers on-site were water damaged and expired; I left mine kit with the facility. Corrective Actions: Obtain a QT-40chemical test kit to ensure the sanitizing solution is maintained at the proper solution.
- Three part sink sprayer nozzles and rest-room toilet fixtures aren't kept clean. Corrective Actions: Maintain non-food-contact surfaces of equipment clean.
- Door handle is missing to the first rest-room (hole in door) and that rest-room's trash can lid is missing. Corrective Actions: Keep toilet room doors closed except when cleaning or during maintenance operations to prevent insect and rodent entrance and the associated potential for the spread of disease. Provide a covered trashcan in the rest-room for female hygiene products.
