Ashburn Health Inspections: Matchbox, Casa Tequila, Bistro Atelier

The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.

ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between Oct. 11 and Oct. 17.

COVID-19 restrictions around the state have disappeared, so health inspectors conduct most restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

    • Date inspected: Oct. 11
    • Total violations: 2
      • Portioned uncovered cups of bean salad available for self service by improperly stored outside of the sneeze guard on the serve line.
      • Wet wiping cloths improperly stored in between use. Wet wiping cloths stored in sanitizer bucket. When tested the sanitizer concentration registered at 0ppm sanitizer.
  • Paisanos Ashburn, 44260 Ice Rink Plaza #109 Ashburn, VA 20147
    • Date inspected: Oct. 12
    • Total violations: 2
      • There isn't sufficient available Chlorine sanitizer in dish machine. Sanitizer is reading 10 parts per million. Maintain 50-100 parts per million in a dish machine using Chlorine sanitizer for proper sanitization.
      • Observed time/temperature control for safety foods (TCS) not properly dated for disposition. Foods not labeled include: sliced deli meat. Ready to eat TCS foods held over 24 hours must be date marked with either made on date or date the food item must be consumed not to exceed 7 days.

    • Date inspected: Oct. 12
    • Total violations: 2
      • Tamales and a container of cooked chicken in the refrigerator was observed with no date markings. Time/temperature control for safety (TCS) foods that are ready to eat (RTE) that are kept longer than 24 hours, must have a date labeling when the food was opened/prepared, or a discard date not to exceed 7 days from the date that the food was opened/prepared. The prepared day shall count as day 1. Date label these items.
      • A container of raw beef cut into strips was observed thawing on the counter at room temperature. When thawing TCS foods, they must be thawed under cold, continuously running water, or under refrigeration that maintains 41F or less. Do not thaw TCS/RTE foods at room temperature.

    • Total violations: 2
      • No hand drying provisions were provided at the hand washing station. Provide hand drying provisions within reach of the hand washing station. Paper towels were placed at the hand washing station.
      • A large bag containing smaller bags of potato chips was found stored in the basin of the hand washing sink. Hand sinks shall be kept clear of any items/obstacles and used for no other purpose but hand washing (no storage of items). Keep hand washing clear so that it may be used at all times. Bag was removed from the basin of the hand washing sink.

      • The prep sink in the kitchen has been equipped with a hand soap dispenser and a paper towel dispenser. Food preparation sinks are not to be equipped with handwashing aids and devices.
      • Marinara sauce date marked 9/18/2023 and chicken nuggets date marked 10/4/2023 noted as held past 7 days (in the refrigeration unit) from the day the commercial made product was opened.
  • Starbucks Coffee C-7, 1 Saarinen Cir. Dulles, VA 20166
    • Date inspected: Oct. 12
    • Total violations: 2
      • Milk registered at 46°F in the front low boy refrigerator in front coffee and drinks prep area. Said unit registered operating at 50°F.
      • Few live fruit flies observed in the dishwashing area service area.
  • Bistro Atelier, 1 Saarinen Cir. Dulles, VA 20166
    • Date inspected: Oct. 12
    • Total violations: 7
      • Proper eating and drinking procedure not followed while prepping food in kitchen in that a food handler was observed eating and drinking from an beverage containers without straw while prepping food in the kitchen. Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items.
      • Soap was not provided at the only hand washing facility in the kitchen, the coffee prep area and at the bar.
      • Ready to eat ham to be sliced observed placed on top of the cardboard box.
      • Raw animal food stored over ready-to-eat food in the refrigeration unit in that unpasteurized raw shell eggs observed placed above containers of cooked onion soup in a shelf in the walk in refrigerator.
      • The drawers of low boy drawer refrigerator infront of the walk in refrigerator observed loose and not closing completely. Said unit observed holding TCS food items.
      • Vacuum packaged tilapia not thawed properly in that vacuum packaged tilapia observed placed in low boy refrigerator in the original package.
      • Live fruitflies observed in the bar area.
  • Dunkin Donuts D-22, 1 Saarinen Cir. Dulles, VA 20166
    • Date inspected: Oct. 12
    • Total violations: 2
      • The handwashing facility not readily accessible for employees to wash hands in that hand sink located at the rear food prep area observed with large bottle of dishwashing soap placed in it, and the handwashing sink in front food service area observed with wiping cloth placed in it preventing access by employees for easy handwashing.
      • Employees' personal care items are not stored in lockers or other suitable facilities in that employees personal belonging observed stored with clean utensils and single service items in shelf in the facility.

    • Total violations: 1
      • Rice cooked the previous day noted as improperly cooled to prevent the growth of harmful bacteria. The rice was prepared on 10/11/2023 the temperature of said rice registered at 44F.
  • Casa Tequila Bar & Grill, 7 Pidgeon Hill Dr Sterling, VA 20165
    • Date inspected: Oct. 12
    • Total violations: 8
      • Raw shell eggs observed stored over ready to eat (RTE) food items in the reach in cooler on the line. Food shall be protected from cross contamination risks by separating raw animal proteins from RTE foods and arranging in a way so that the RTE foods are not subject to spills, leaks, splash or other avenues of cross contamination. Person in charge (PIC) moved the eggs to the bottom shelf.
      • A large container of beef was temped at improper cold holding temperatures. Container temped at 49F with a prepared date of 10/11. Time/temperature control for safety foods (TCS) must be maintained at 41F or less. PIC voluntarily discarded the entire container of beef.
      • A spray bottle containing a blue liquid cleaning product was found with no identifying label. Toxic materials/cleaning supplies taken from bulk quantities must be labeled with the common name of the material on the working container. Label all spray bottles with their contents.
      • Several packages of salmon in reduced oxygen packaging (ROP) were found thawed with the vacuum still intact. When thawing fish that is in ROP, the package must be poked, cut, slit or otherwise break the vacuum to introduce oxygen so that harmful anaerobic bacteria cannot multiply before thawing. Break these vacuums before the thawing process begins. PIC broke the vacuum seal on all the salmon packages.
      • A container of margarita mix was found stored directly on the floor, underneath the three basin sink at the bar. Food shall be protected from contamination by storing at least 6" off the floor. PIC moved the margarita mix to a shelf.
      • Mixing spoons found stored in a container of stagnant water behind the bar. In-use utensils that are stored between uses must be stored on a clean dry surface, in water that is 135F or more, or in constantly running with enough velocity to wash away food particles. PIC removed the utensils and replaced in a clean, dry container.
      • No test strips to test the chlorine sanitizer for the low temperature dish machine behind the bar could be provided. Test strips, or other testing device must be provided and accessible to monitor the concentration of sanitizer for the low temperature dish machine. Procure test strips ASAP.
      • Soil debris (black) was observed on the deflector of the ice machine. Non food contact surfaces shall be kept clean and clear of an accumulation of soil debris. Clean the ice machine.
  • Homewood Suites Continental Breakfast, 44620 Waxpool Rd Ashburn, VA 20147
    • Date inspected: Oct. 13
    • Total violations: 5
      • There should be available a means to verify that food employees have been trained on the signs, symptoms, and exposures of foodborne illnesses and who to report to. All food employees should be trained on the signs symptoms of foodborne illnesses and who to report to.
      • Observed improper cold holding temperature of cut melon in an ice bath on the counter. Foods held cold should be maintained at 41F or below.
      • Observed ice machine leaking down the side of ice machine onto the floor and pudding on the floor. Equipment should be maintain in good repair.
      • Observed no test strips for the quaternary sanitizer at 3-compartment sink. Facilities should have a means to test sanitizer concentration.
      • Observed a large amount of juice buildup under the white plastic where the spouts of juice. Non food contact surfaces should be cleaned at a frequency necessary to prelude accumulation of soil residues.

    • Total violations: 3
      • The person in charge has failed to ensure employees are complying with the requirements of the Regulations, such as avoiding the storage of the raw food animal origin above the ready to eat food and employees tasting food while cooking.
      • Employees observed tasting food while working in the kitchen.
      • Observed raw shell eggs stored over ready to eat food in cooler. Prevent potential cross contamination by not storing raw animal foods over ready to eat foods.

      • Person in charge failed to set up the 3 compartment sink properly. Employee was setting up the wash basin with soap and sanitizer.
      • There is no soap at the hand sink to wash hands. There must be soap available at hand sink for proper hand washing at all times.
      • There are no paper towels at the hand sink to dry hands. There must be paper towels available at hand sink for proper hand drying at all times.
      • Observed raw chicken stored over ready to eat food (watermelon) in cooler. Prevent potential cross contamination by not storing raw animal foods over ready to eat foods.
      • Observed chicken hot holding at improper temperature (117F). Time/temperature control for safety (TCS) foods must be hot held at 135F or above to prevent growth of harmful bacteria.
      • No test strips to test sanitizer. Provide test strips.

      • Observed sausage and white sauce cold holding at improper temperature above 45 degrees(F). Time/temperature control for safety (TCS) foods must be held at 41F or below to prevent growth of harmful bacteria.

      • There are no paper towels at the hand sink to dry hands. There must be paper towels available at hand sink for proper hand drying at all times.
      • Observed boba tea hot holding at improper temperature (130F). Time/temperature control for safety (TCS) foods must be hot held at 135F or above to prevent growth of harmful bacteria.
      • Food employee with artificial nails or fingernail decorations other than nail polish observed handling exposed food.
      • The light bulb in the walk in cooler is out.

    • Date inspected: Oct. 16
    • Total violations: 3
      • Kitchen manager could not locate proper documentation of an employee health policy showing kitchen staff are properly trained to report symptoms of illnesses transmittable through food. Manager must document staff training.
      • One hand sink does not have running water. There are two working sinks in the kitchen. Person in charge is going to place a work order to fix the sink.
      • Person in charge could not find test strips. Person in charge will order more test strips.

      • Food employees or conditional employees were not aware or informed of their responsibilities in reporting symptoms or diagnosis of diseases transmissible through food.
      • Observed raw chicken stored over ready to eat food (raw vegetable) in cooler. Prevent potential cross contamination by not storing raw animal foods over ready to eat foods.
  • Capital One Lounge, 1 Saarinen Cir Sterling, VA 20166
    • Date inspected: Oct. 16
    • Total violations: 9
      • Hand sink near the dish machine observed filled with stainless steel scrubbing pads and the hand sink at the coffee bar was observed with a drying rack laid across it. Hand sinks are to be utilized for hand washing only.
      • EHS observed the dishwasher handle soiled wares with rubber gloves. They loaded the dish machine with soiled dishes and then unloaded the dishmachine and handled clean dishes with the same rubber gloved hands.
      • When tested, the bar dish machine sanitizer concentration registered at 0ppm. Glassware observed not sanitized due to 0ppm sanitizer.
      • Large lexan container of mashed potatoes prepared the day before noted as improperly cooled to prevent the growth of harmful bacteria. Mashed potatoes registered at 45F. EHS observed an accumulation of condensation on the interior lid of the lexan container.
      • This facility utilizing a form of time control for the cold foods on the self service "buffet" according to the person in charge if a TCS food "visually" looks like it needs to be replaced on the line it is. TCS foods are unable to maintain temperature control at the cold stations at the self service line. Foods are individually plated and placed on a refrigerated surface. According to the PIC TCS foods are rotated out every 10 minutes. TCS foods are not marked with a time stamp when they are removed from temperature control (coolers in the back kitchen) and placed under time control at the self service line. The PIC stated that they do have an electronic monitoring system for time control but it is currently having connectivity issues.
      • Packaged foods (sandwiches, salads, desserts) available for guest self service (grab and go coolers at front of facility) are missing the following from the label: - Net weight statement - The name and place of business of the manufacturer- address - sub- ingredients - allergen declaration.
      • Facility unable to locate high temperature dish machine thermometer for the high temperature dish machine. In hot water mechanical warewashing operations, an irreversible registering temperature indicator shall be provided and readily accessible for measuring the utensil surface temperature.
      • Facility unable to locate sanitizer test strips for the low temp bar dish machine. Employees unable to test sanitizer concentration of the bar dish machine.
      • The wall near the chemical storage shelf and rolling trash receptacle is not smooth, durable, easily cleanable, and/or nonabsorbent. It appears to be a raw wood wall.

    • Date inspected: Oct. 17
    • Total violations: 3
      • Raw beef patties found stored over mixed ready to eat (RTE) cheese and RTE marinara. Food shall be protected from cross-contamination by separating raw animal proteins from RTE foods. Beef patties were moved to the bottom shelf, beneath the RTE foods.
      • Several spatulas were found stored with chips, splits, cracks and melted portions on them. Food contact surfaces must be free of chips, breaks, cuts or other impediments so that they are able to be properly sanitized. Person in charge voluntarily discarded the compromised utensils.
      • The handwashing sink at the pizza line has a moderate leak. Plumbing systems shall be maintained in good repair. Repair the sink so that it no longer leaks.
  • Matchbox One Loudoun, 44720 Thorndike St Ashburn, VA 20147
    • Date inspected: Oct. 17
    • Total violations: 9
      • Hand sink in the front pizza prep area observed without paper towels.
      • The handwashing station being used for purposes other than washing hands in that the only handwashing sink at the bar area observed being used as dump sink. Said sink observed with strainer placed in it.
      • Severely dented can of black beans in brine observed stored along with other not dented cans of black beans in brine in a shelf in dry storage area.
      • Chemical dishwasher in the kitchen is not sanitizing adequately after wash and rinse cycle in that when tested with the sanitizer test strips, said test strip did not change color to the acceptable range.
      • The chicken was not cooked to a sufficient temperature and time to eliminate pathogenic bacteria in that grill chicken was registered at 160°F when the chef stated that it was fully cooked.
      • Improper hot holding in that spinach dip and crab dip registered at 104°F in hot holding table in front food prep area. Said hot holding table observed with low water just touching the base of the containers holding the TCS food items.
      • Several food items such as cooked mushroom, mixed cooked veggies, fresh mozzarella, margarita sauce registered between 44°F-47°F in front pizza prep area. Lower section of three refrigeration units registered operating between 47°F.
      • In-use utensils improperly stored between use in use ice scoop observed stored in ice with handle touching the ice.
      • The pipe in three compartment sink on second floor dishwashing area observed leaking.
  • Cafesano Restaurant, 21305 Windmill Parc Dr #100 Sterling, VA 20166
    • Date inspected: Oct. 17
    • Total violations: 3
      • Observed raw shell eggs stored over ready to eat food in cooler. Prevent potential cross contamination by not storing raw animal foods over ready to eat foods.
      • Alfredo sauce and Marinara sauce are improperly cooled to prevent the growth of harmful bacteria. They both were made last night and has been cooling for more than 6 hours. Cool time/temperature control for safety (TCS) food(s) from 135F to 41F within a total of 6 hours and from 135F to 70F within 2 hours.
      • Chemical spray bottle observed without the proper labeling. All working containers of toxic materials should have a label with the common name of the material in it.
  • KFC / Taco Bell, 45960 Denizen Plaza Sterling, VA 20165
    • Date inspected: Oct. 17
    • Total violations: 2
      • Observed the Soap Dispenser at the Handwashing station by the drive through to be empty.
      • Observed that the 2 tiles behind the Henny Penny Fryers are detaching from wall, it appears to have pressure from debris/grease behind them.
