Ashburn Health Inspections: Moldy Mushrooms, Mouse Droppings
The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.
ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between Oct. 18 and Oct. 24.
COVID-19 restrictions around the state have disappeared, so health inspectors conduct most restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

- Gumagumalu Kitchen, 506 Shaw Road #331 Sterling, VA 20166
- Date inspected: Oct. 18
- Total violations: 6
- Cooked eggplant and mushroom in the two door reach in refrigerator observed with mold growth. As per the person in charge (PIC) said food has been in said unit for four days.
- Unwrapped or uncovered food in the shelves during storage. Facility shall store food in air tight containers in shelves.
- Raw animal food stored over vegetables in the refrigeration unit in that trays of raw marinated chicken and fish observed placed on top of vegetables on the shelf in walk in refrigerator in kitchen.
- Date marking not provided to prepared temperature control for safety (TCS) food such as cooked paneer biryani, cooked vegetables in two door reach in refrigerator. As per the PIC said food were prepared four days prior and being held in the reach in refrigerator.
- Few mice droppings observed on shelves and in dry goods storage room. As per the person in charge (PIC), the facility provides regular pest control treatment.
- Several unused old equipment observed dirty with debris, dust, mice droppings and dead flies in and on them.

Store raw animal foods in a manner that prevents cross contamination. Employee relocated food to protect from cross contamination. Observed several food items stored uncovered in cooler. Employee covered foods to prevent contamination.
- Observed potatoes hot holding at improper temperature (122F). Time/temperature control for safety (TCS) foods must be hot held at 135F or above to prevent growth of harmful bacteria. - The person in charge is unaware that both dish machines are not sanitizing in the facility, was not aware of how the sanitizer test strips are used for said dish machines, and did not know proper cold holding and hot hold temperature requirements for TCS (time temperature control for safety) foods,
- Open drink cup improperly stored on the clean plateware shelf in the kitchen over container of chips.
- EHS observed a knife stored in between two low boy coolers (table top prep units). This area is a hard to reach area for cleaning and sanitizing. The prep tables are fixed units. The employee took the knife and started slicing an avocado for a salad. Foods shall only come into contact with equipment and utensils that have been cleaned and sanitized.
- Both low temperature dish machines in the facility sanitizer concentration registered at 0ppm when tested. Wares not sanitized after cleaning and before use.
- TCS foods hot holding at improper temperatures at the line stove. Employees are placing containers of TCS foods on a sheet tray over the stove for easy access during the lunch rush. TCS foods registered at- burgers 100F, Philly steak beef 107F, chicken breasts 108F cooked onions 100F.
- This is a repeat violation. The menu is printed without a consumer advisory disclosure and reminder. The burger on the menu is cooked per the customer's request and undercooked fried eggs are also offered per the customer's request.
- Improper methods used to thaw Reduced oxygen packaged salmon. Salmon left in the reduced oxygen packaging at refrigeration temperatures.
- Facility was not able to locate sanitizer test strips for the low temperature dish machine. Food employees unable to test sanitizer concentration at the low temperature dish machines.

- Observed corn bread that was not covered in the back on a prep table. Ready to eat foods should be cover from cross contamination. Person in charge (PIC) covered the corn bread.
- Observed can opener blade that is dull and dirty. Food contact surface should be clean to touch and sight. The can opener blade needs to be replaced.
- Observed improper hot holding temperature of pork ribs in warmer (125). Hot holding foods need to be at 135F or above if under 135F it will need to be reheated to 165F. PIC reheated the pork ribs to 183F.
- Observed hard salami that its best by date was 10/12/23. The date marked by food establishment should not exceed a manufacturer's use by date if the manufacturer determined that date based on food safety.
- Observed Cole slaw that was tightly covered during cooling process (45F). To help with the cooling process the container needs to be loosely covered or uncovered. PIC uncovered the coleslaw to help the cooling process (41F).
- Observed rusted shelves in walk in cooler, rusted bottom shelf prep table and dirty ice guard in ice machine. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
- Observed three compartment sink and there is no cold water. Plumbing system should be maintained in good repair.
- Observed floor in the walk in cooler not in good repair and had water on the floor. The physical facilities shall be maintained in good repair.

- There is no available Quaternary sanitizer in sanitizer sink being used to wipe down equipment/sanitizing food surfaces after washing. Sanitizer is reading 0 parts per million. - Cooked pizza for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve or discard by time.

- When requested, the establishment was only able to produce one copy of an employee health reporting form (1B) signed by an employee. Establishments must have verifiable means that all employees have been trained in the signs and symptoms of food borne illness and how to properly report them. These documents will be requested and reviewed at every inspection and must be available for review. Leave these documents within the facility at all times.
- The inside bin of the ice machine (food contact surface) as well as the deflector (Non-food contact surface) was observed with soil debris accumulation (black). 2 knives stored on a magnetic knife strip observed with heavy food debris on them. Food contact surfaces must be clean to sight and touch. Clean the ice machine so that no debris is present/visible and clean at a frequency that precludes the accumulation of soil debris. Knives were removed from strip and placed in the three basin sink to be rewashed.

- A food service worker was observed failing to wash their hands after handling dirty sheet pans and before handling clean pans. Corrected on site - the Person in Charge spoke to the food service worker, who then washed their hands and rewashed the sheet pans handled with dirty hands. Train all staff to wash their hands when changing tasks, such as after handling dirty wares and before handling clean wares.
- All hand sinks are not provided with an approved hand drying provision. At multiple hand sinks a roll of paper towels without a dispenser is provided. Correct by providing a supply of individual paper towels or installing and using a paper towel dispenser.
- According to the date mark of 09/26/2023, the house made curtido observed in the walk-in cooler should have been discarded on 10/02/2023. Corrected on site - when this item was brought to the attention of the Person in Charge they discarded the food. Promptly discard Temperature Control for Safety foods at the end of the seven day cold holding period.

- Employee's coffee mug observed in multiple locations throughout the kitchen. Food employees may only eat, drink, or use tobacco in a designated area like a dining room or office area. - Observed a box of raw chicken stored over ready to eat food (onion) in cooler. Prevent potential cross contamination by not storing raw animal foods over ready to eat foods.
- There is a gap between the back door and the floor which can allow the insect to enter the facility.
