Ashburn Restaurant Inspections: Sushi Queen, King Pollo, Rai's Pizza

The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.

ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between March 1 and March 7.

Most COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

    • Date inspected: March 1
    • Total violations: 4
      • Observed 2 of the handwashing sinks to have empty paper towel dispensers.
      • Observed 5 sets of in use tongs stored on 2 separate oven handles. Observed Ice Scoop lying flat in ice bin with handle contacting potable ice.
      • Observed Ice Machine’s Guard inside machine not clean and has pink residue.
      • Observed one soiled mop not hung to dry in mop sink area out of 3.

    • Date inspected: March 1
    • Total violations: 1
      • A container of Bolognese was observed covered, while cooling in the walk-in cooler. When cooling foods, do not cover until completely cooled, then cover to protect from outside sources of contamination. Person in charge (PIC) uncovered the Bolognese and moved to the freezer to rapidly. Corrected.

    • Date inspected: March 1
    • Total violations: 3
      • Time/temperature control for safety (TCS) foods must be hot held at 135°F or above to prevent growth of harmful bacteria in that marinara sauce registered at 69°F on hot holding unit that was not turned on. As per the person in charge (PIC), said food was taken from the cooler and placed in said location an hour ago.
      • Time/temperature control for safety (TCS) foods must be held at 41°F or below in that ham, cut tomatoes and cheese registered between 46°F-48°F in the top section of sandwich prep cooler refrigeration unit in food prep area. Said unit not adequately cooled to maintain the TCS food items temperature at or below 41°F.
      • Time/temperature control for safety (TCS) foods must be held at 41°F or below in that ham, cut tomatoes, raw shell eggs, chicken wings and cheese registered between 46°F in the walk in refrigeration unit in the kitchen. Said unit not adequately cooled to maintain the TCS food items temperature at or below 41°F.

    • Total violations: 5
      • The hand sink in the kitchen was blocked by two boxes, obstructing use of the hand sink. Hand sinks must be available and easily accessible at all times to food employees. Person in Charge (PIC) removed the boxes so that the hand sink is unobstructed. Corrected.
      • Two packages of raw beef observed stored over a drawer of Roma tomatoes in the reach-in refrigerator. Food shall be protected from cross-contamination by separating raw animal proteins from ready-to-eat (RTE) foods and arranging in a manner that mitigates the risk of contamination from drippings. PIC removed the two packages of raw beef to the low boy reach in cooler. Corrected.
      • A tray of tamales made the night of 2/27 was observed cold holding at 43F, above the required maximum of 41F. Time/Temperature control for safety foods (TCS) that are RTE must be maintained at 41F or less to inhibit the growth of harmful bacteria. EHS recommends that these food items not be sold to the public. Owner agreed to take home at the end of the day.
      • A spray bottle containing a diluted bleach solution was found with no identifying information. Toxic materials that are taken from bulk supplies must be labeled with the common name on the working container. PIC labeled the spray bottle "bleach". Corrected.
      • The reach-in refrigerator does not have a thermometer or other temperature reading device that monitors ambient air temperatures. A thermometer or other sensor to monitor ambient air temperature within a refrigeration unit is required. Provide a thermometer for the refrigerator.

      • Observed the Hand Washing Sink next to the Office to be turned off due to the sensor malfunctioning to distribute automatic stream of water.

    • Date inspected: March 1
    • Total violations: 3
      • Facility did not have documentation that current employees had undergone training of the signs and symptoms of food borne illness and how to properly report them. Written/documentation that employees have been trained in the signs and symptoms, how to report them and their significance to food safety shall be provided upon request. EHS printed out form 1B (x5). All employees read and signed this form during the inspection. Discussed with Manager that all food employees must read/sign this document. Keep within the facility.
      • The facility did not have a written policy/procedure to properly clean up any vomit/diarrheal events. A written policy/procedure employees may follow to properly clean-up these events is required. EHS printed NOROVIRUS clean-up worksheet.
      • EHS observed food debris in the hand sink. Hand sinks are to be used solely for hand washing and no other purpose. Do not use the hand sink to dump.
  • Subway, 45985 Regal Plaza #120 Sterling, VA 20165
    • Date inspected: March 1
    • Total violations: 4
      • No written policy/procedure directing the proper clean-up of vomit/diarrheal events. Establishments must have a written policy/procedure that employees can follow to properly clean-up vomit/diarrheal events. EHS printed the NOROVIRUS clean-up worksheet in both English & Spanish. Corrected.
      • The hand sink next to the prep table in the back kitchen was observed with no signage reminding employees to wash their hands. All hand sinks that are utilized by food employee must include signage reminding employees to wash their hands. Add handwashing signage to this hand sink.
      • A personal cell phone stored on the prep table and an employee's bag (purse) was observed stored on top of dry food products in dry storage. Foods and food contact surfaces shall be protected from sources of contamination by separating personal items and storing them in areas designated for employees. Person in charge moved the phone and purse to the employee area. Corrected.
      • No testing device/strips to test/monitor the concentration of Quat sanitizer used at the three basin sink. A testing device/strip must be provided in order to test and monitor the concentration of the sanitizer. PIC stated that test strips will be delivered tomorrow. EHS gave strips to PIC to use until delivery. Corrected.
  • Mona's Lebanese Cafe / Guilford Station Arts Club, 1006 Ruritan Circle Sterling, VA 20164
    • Date inspected: March 1
    • Total violations: 3
      • The hand sink in the kitchen is only providing a trickle of cold water. The flow of the water and temperature of the water needs to be increased (at least 100F) so employees can properly wash their hands. This is a repeat violation.
      • This facility does not have a metal stem food thermometer to monitor food temperatures. All food establishments shall have a metal stem food thermometer to monitor proper food temperatures. This is a repeat violation.
      • EHS observed food storage in an unapproved building on property. EHS observed the owner of this facility entering an adjacent building upon arrival to the facility. EHS asked to enter said building to observe what was inside. EHS observed two stand up freezers and a refrigerator and dry storage of single service containers, a stand up mixer, as well as an unused oven. This building was not approved for food storage and was never disclosed to the EHS in previous inspections. When asked in previous inspections, the owner has stated that there was no food storage in said building. The building has an unfinished floor that the owner is covering with flattened cardboard boxes as floor mats. The floor is not easily cleanable. EHS also observed gaps in the windows in which pests or vermin could enter.

    • Date inspected: March 1
    • Total violations: 6
      • Person in charge could not provide documentation that food employees have been trained on the signs, symptoms, and exposures to foodborne illnesses and who to reported.
      • Observed hand sink by 3-compartment sink with no soap. Hand sinks should have soap available at all times for employees use.
      • Observed mop bucket in front of hand sink and buckets in the hand sink by the 3-compartment sink. Hand sink should be available at all times for employees use.
      • Observed knife in holder with food residue on the blade and the base of slicer were food would go after being sliced with food residue. Food contact surfaces should be clean to sight and touch.
      • Observed pizza roll that was kept for moved that 7 days. Pizza roll must be served, sold or discarded within 7 days of preparation including the date of preparation of the first prepared ingredient.
      • Observed 3 types of pizza on display facility uses Time as a Public Health Control (TPHC) without a time when pizza was placed in the display.

    • Date inspected: March 2
    • Total violations: 5
      • 3 Compartment Sink found to have a sanitizer level of 0ppm and a broken dispenser with that being used in place of the Hobart Dishwashing Machine in state of disrepair. Unaware of required test strips and procedures.
      • 0 ppm Quat being used at 3 basin sink. Utensils are not being properly sanitized.
      • An Unmarked Spray Bottle was found containing Lysol Multi Surface Cleaner.
      • Quaternary ammonia test strips for testing sanitizer at 3 compartment sink not available.
      • Observed the Spray Hose to be hanging below the rim of the sink/flood level, down to the drain.

      • Bowl used as scoop in salsa (ready to eat). Provide scoop with handle. Ready to eat foods cannot be handled with bare hands.
      • There are no paper towels at the hand sink to dry hands. There must be paper towels available at hand sink for proper hand drying at all times.
      • There is dried food residue on slicer after it has been washed/rinsed/sanitized. Food contact surfaces shall be properly cleaned and sanitized after using.
      • The beans were not reheated to a sufficient temperature and time to eliminate pathogenic bacteria. Employee reheated food to 165F before placing back for hot holding.

    • Total violations: 6
      • The person in charge failed to explain the relationship between food safety (proper cold holding temperatures). There was a cooler completely stocked with foods while holding at 57F.
      • Observed employees directly contacting exposed cooked crab with their bare hands in a manner that could contaminate the food. Employee stopped, washed hands, and put on gloves.
      • There is no soap at the hand sink to wash hands. There must be soap available at hand sink for proper hand washing at all times.
      • Observed raw chicken stored over ready to eat onions and food in cooler. Prevent potential cross contamination by not storing raw animal foods over ready to eat foods. Observed several food items stored uncovered in cooler. Prevent potential cross contamination by not storing raw animal foods over ready to eat foods. Observed batches of tempura sitting uncovered, directly on top of on another with bottom of container in foods and directly under paper towel holder where it would be contaminated by water run off from handwashing.
      • In use knives (3) stored slid down in between equipment where there is no way to clean. Knives are also stored in the dirty location for use without be cleaned after use.
      • Rice for which time rather than temperature is being used as a control is not properly labeled.
  • Cafe Hyderabad, 22621 Amendola Terrace 160 Ashburn, VA 20148
    • Date inspected: March 6
    • Total violations: 5
      • Management must be aware and inform all employees of their responsibility in reporting symptoms or diagnosis of diseases transmissible through food. Manager was unable to provide a signed copies of employee health reporting agreement.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Time/temperature control for safety (TCS) foods must be held at 41°F or below in that a container of cut tomatoes registered at 52°F in top section of low boy refrigeration unit. Said unit registered operating at 50°F.
      • A Variance request was not submitted prior to engaging in preparing yogurt at the facility.
      • Three compartment sink observed with only spraying hose.
  • Bungalow Lake House, 46116 Lake Center Plaza Sterling, VA 20165
    • Date inspected: March 6
    • Total violations: 4
      • The low temperature dish machine behind the main indoor bar, and the dish machine used by servers in the cigar bar both registered at 0ppm chlorine sanitizer. Drinkware that is cleaned in these dishwashers is not being sanitized. Food contact surfaces shall be sanitized, before use, after cleaning. Per Manager, staff will use the dish machine at the server station at the main bar, and the bar dish machine at the cigar bar until this equipment is sanitizing correctly. Corrected.
      • Several spray bottles holding a clear liquid cleaning agent with no identifying information were found throughout the establishment. When using toxic materials taken from bulk supplies, the common name of the material must be marked on the working container. PIC had employee label all the spray bottles during the inspection. Corrected.
      • Spatulas and tongs were found stored in a metal container of stagnant water on the line. Several knives were observed stored/wedged between coolers/equipment on the line. Utensils that are in-use must be stored in between uses on a clean, dry surface, in water that is 135F or more, or in cold running water that has enough velocity to wash away food particles. PIC removed utensils to be rewashed.
      • Two spatulas were found stored, available for use with portions that were cracked, chipped and melted. Food contact surfaces of utensils shall be free of chips, cracks, pits, or other similar imperfections so that all surfaces are easily cleanable and able to be thoroughly sanitized. PIC voluntarily discarded both spatulas. Corrected.
  • McDonald's, 20961 Southbank Street Sterling, VA 20165

    • Total violations: 2
      • No hand towels or other means to properly dry hands was observed at the hand sink located at the back portion of the assembly line. All hand sinks that are utilized by food employees must have a means to properly dry hands after washing. PIC refilled the dispenser with a fresh supply of paper towels. Corrected.
      • No cleanser for hand washing was observed at the hand washing sink located at the drive thru window. All hand washing sinks that are utilized by employees must be supplied at all times with a hand washing cleanser. Person in charge (PIC) had the soap dispenser refilled during the inspection. Corrected.

    • Date inspected: March 7
    • Total violations: 2
      • Sliced beef and diced chicken observed in the small reach in cooler cold holding food above 41 degrees. Cold hold food at 41 degrees or below.
      • Chemical spray bottles observed stored above the pizza dough in the kitchen. No toxic material should be stored above the food.

    • Date inspected: March 7
    • Total violations: 2
      • An open glass used for an employee beverage was observed with no cover. Employees may drink from containers that are closed (Lid) and are handled in a way that prevents contamination to utensils, food contact surfaces etc. Person in charge (PIC) removed the glass and replaced with a to go cup with lid and straw. Corrected.
      • Several utensils (stirrers) were observed stored in a dipper well that was not on, in stagnant water. When storing in-use utensils in between uses, store on a clean dry surface, in water that is 135F or more, or in cold running water with enough velocity to wash away loose food particles. PIC removed the utensils to be rewashed. Turn on dipper well if utensils are to be stored inside.
