Cobwebs Reported On Pizza Dough Roller Machine: Health Inspections
Health inspectors reported finding 18 violations at one Fredericksburg restaurant, including dirty utensils and cobwebs on equipment.

FREDERICKSBURG, VA — Rappahannock Area Health District inspectors visited 20 food establishments in Fredericksburg between Sept. 5-13, finding health code violations at 11 of them.
At one food establishment, an inspector reported 18 violations, including seeing cobwebs on the pizza dough roller, a dirty can opener, a microwave ove that needed cleaning and small flying insects in the dishware washing area.
All of the equipment needed to be cleaned and sanitized, according to the report.

"The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises," the inspector wrote.

Foods were voluntary discarded. EHS recommended to the person in charge adding ice to the bottom of the ice chest cooler. - Observed stainless steel food containers stacked while wet after sanitizing on the shelving units near the dishmachine. Discussed requirements with the person in charge. Containers were relocated to the warewashing area to be washed and sanitized. (corrected during the inspection). Corrective Actions: After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food.
- Observed a water hose connected to the main water line that was attached to a Y connector and stored at the mop sink basin. During the inspection, the person in charge cut the hose to create the required air gap between the hose and flood level rim. (corrected during the inspection). Corrective Actions: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bibb (threaded faucet) if a hose is attached or on a hose bibb if a hose is not attached and backflow prevention is required by law by: 1. Providing an air gap as specified under 12VAC5-421-2200;P or 2. Installing an approved backflow prevention device as specified under 12VAC5-421-22.

- Observed two food employees wash their gloved hands and then proceed to go back to the cookline. EHS spoke to the employees and the person in charge. The food employees removed their gloves, washed their hands, and donned new gloves before returning to the cookline. Corrective Actions: If used, single-use gloves shall be used for only one task such as working with ready-to-eat food or with raw animal food, used for no other purpose, and discarded when damaged or soiled, or when interruptions occur in the operation.
- Observed the inside plastic shield and the side (inside) of the ice machine soiled with a black mold-like substance. Corrective Actions: Surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned at a frequency specified by the manufacturer or absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold.
- Observed 7 small bags of ramen noodles in the walk-in cooler cold holding at 47°F. The noodles were in the cooler overnight. All bags were voluntarily discarded. Corrective Actions: Time/temperature control for safety food shall be maintained at 41°F or below.
- Observed three food employees in the kitchen without hair restraints. Two were preparing food and one was washing dishes. The person in charge spoke to them and they restrained their hair. Corrective Actions: Food employees shall wear hair restraints, such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles.
- Both faucets and their handles at the three-compartment sink are lose. Corrective Actions: Equipment shall be maintained in a state of repair and condition that meets the requirements.
- Observed the inside of the dish machine doors soiled. Corrective Actions: Clean the dish machine at a frequency necessary to prevent recontamination of equipment and utensils and to ensure that the equipment performs its intended function.
- The person in charge was unable to provide a valid test kit for the chlorine sanitizer. Corrective Actions: A test kit or other device that accurately measures the concentration in mg/L (ppm) of sanitizing solutions shall be provided and readily accessible for use.
- Observed the floors and walls in the kitchen area soiled. Corrective Actions: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

- The person in charge was unable to provide proof of training or a policy outlining the big 6 foodborne illnesses and their common symptoms. EHS provided a sample copy of an employee health policy to be reviewed, signed, and dated by all food employees. Corrective Actions: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food.
- The following foods were being held below 135°F in the first steam table wells on the right: 1. pulled chicken was at 102°F; and 2. pulled pork was at 112°F. The items had only been in the unit for 1 hour out of temperature, they have logs. The items were quickly reheated. The chicken was reheated to 169°F, and the pork to 180°F. The person in charge advised it was found that the plug had been partially pulled out of the socket. Corrective Actions: Time/temperature control for safety food shall be maintained at 135°F (57°C) or above.
- Observed multiple plastic containers stacked on clean dish storage racks while wet. Corrective Actions: After cleaning and sanitizing, equipment and utensils shall be air dried or used after adequate draining.
- Observed the following non-food contact surfaces soiled: 1. the outside of the high temperature dish machine; and 2. the two hand sinks in the kitchen area; 3. sheet trays and metal pans had carbon build-up. Corrective Actions: Non-food contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
- Observed the inside of the exhaust system above the high temperature dish machine and the vent in the dry storage area behind the smokers soiled. The person in charge advised the cleaning service for the exhaust vent for the dish machine is due this month. Corrective Actions: Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials.

Corrective Actions: Provide a written plan for employees to follow when responding to a vomiting or diarrhea events with discharge of vomitus or fecal matter onto food surfaces that includes the procedures for cleaning and sanitizing affected areas, disposing of the discharge and contaminated food items, and protecting employees while they respond to a vomting or diarrheal event.
- Observed mold on an uncut bell pepper in the walk-in cooler. Bell pepper was discarded. Corrective Actions: Food shall be safe, unadulterated, and, as specified in 12VAC5-421-890, honestly presented.
- Observed several containers of food uncovered in the walk-in cooler. Containers were covered during the inspection. Corrective Actions: Foods shall be protected from contamination by being stored in packages, covered containers or wrappings unless being cooled.
- Observed cobwebs on the pizza dough roller machine. Machine was cleaned during inspection.Corrective Actions: Equipment food-contact surfaces and utensils shall be clean to sight and touch.
- Observed the can opener in need of cleaning. Can opener was removed for cleaning. Corrective Actions: Clean and sanitize these surfaces for food contact.
- Observed the interior of the microwave in need of detailed cleaning .Corrective Actions: The food-contact surfaces of in-use cooking and baking equipment shall be cleaned at least every 24 hours. This section does not apply to hot oil cooking and filtering equipment if it is cleaned as specified in 12VAC5-421-1780 D 6.
- Observed no system of date marking system of TCS foods in the walk-in cooler. Corrective Actions: Ensure that all commercially processed RTE time/temperature control for safety (TCS) foods are date marked once opened. The food must be served, sold or discarded within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
- Observed two chemical spray bottles without common brand name. Brand name was written on bottles. Corrective Actions: Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.
- Observed outer openings at the top of the back screen door. Corrective Actions: Except as specified in subsections B through E of this section, outer openings of a food establishment shall be protected against the entry of insects and rodents by:1. Filling or closing holes and other gaps along floors, walls and ceilings. 2. Closed, tight-fitting windows. 3. Solid self-closing, tight-fitting doors.
- Observed several small flying insects in the ware wash area. Corrective Actions: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies. 2. Routinely inspecting the premises for evidence of pests. 3. Using methods, if pests are found, such as trapping devices or other means of pest control. 4. Eliminating harborage conditions.
- Observed containers of foods on the floor of the walk-in cooler. Foods were relocated.Corrective Actions: Except as specified in subsections B and C of this section, food shall be protected from contamination by storing the food: 1. In a clean, dry location. 2. Where it is not exposed to splash, dust, or other contamination. 3. At least 6 inches (15 cm) above the floor.
- Observed a water on the floor in the beverage area. Observed a leak at the filter of the soda machine. Corrective Actions: Equipment shall be maintained in a state of repair.
- Observed the chemical test strips for sanitizer expired. Corrective Actions: A test kit or other device that accurately measures the concentration in mg/L (ppm) of sanitizing solutions shall be provided and readily accessible for use.49: 1800 Observed the following non-food contact surfaces in need of detailed cleaning: 1) interior and exterior of all equipment. 2) gaskets of reach in coolers. 3) soda machine up splash. Corrective Actions: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
- Observed the following throughout the facility in need of detailed cleaning: walls, floors, and ceiling tiles/vents, Corrective Actions: The physical facilities shall be cleaned as often as necessary to keep them clean.
- Observed 20-foot candles on the cookline. Corrective Actions: Provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor
- Observed the hood filters over the cookline in need of cleaning. Corrective Actions: Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials.

- Observed a food employee wearing disposable gloves while handling cans of food and using the can opener. Employee proceeded to touch ready to eat cherry tomatoes without changing gloves. EHS discussed with the employee and PIC proper glove use . During the inspection the employee removed the soiled gloves and washed their hands before donning knew gloves. Corrective Actions: Discard gloves once soiled.
- Observed 30 containers of yogurt on the shelf above the serving line between 68-70 degrees . Through discussion with the person in charge the yogurt was removed from the walk-in cooler at 8:15am . During the inspection yogurt was voluntary discarded. Corrective Actions: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.

- Observed a non kitchen staff employee enter the kitchen without wearing a hair restraint and started scooping ice from the ice machine into a cooler. EHS discussed the following with the non kitchen staff employee and the person in charge : 1.Hair restraint must be worn when entering the kitchen 2. Wash hands when entering the kitchen before touching equipment or untenils. Corrective Actions: Persons unnecessary to the food establishment operation are not allowed in the food preparation, food storage, or ware washing areas of the commercial establishment. Only approved tours or brief visits may be authorized by the Person in Charge or certified food manager is steps are taken to ensure that exposed food, clean equipment/utensils, clean linens, and single service items are protected from contamination.
- Facility does not have a certified food protection manager at this time. Corrective Actions: Ensure at least one person in charge of the establishment acquire a valid FPMC. Certification must be available for observation by the public.

What Violations Do VDH Inspectors Report?
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.
Sometimes, the follow-up inspection only considers the most important "priority" violations.For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation.Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. It can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or officials can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."
