Fredericksburg Health Inspections: 17 Violations At 1 Facility

The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Dec. 19 and Dec. 21.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.

It can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or officials can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Curitiba Art Cafe, 919 Caroline Street Fredericksburg, VA 22401
    • Date inspected: Dec. 19
    • Total violations: 5
      • Observed a sanitizer bucket and wiping cloths sitting in the basin of the handsink by the 3 compartment sink. During the inspection the bucket and cloths were relocated.
      • 0ppm were observed in the sanitizing solution of the 3 compartment sink. During the inspection the solution was remade to 50ppm.
      • Observed the interior lip of the ice machine is soiled and requires cleaning.
      • Observed the following equipment in poor repair: 1. handles of the white reach in freezer of the basement 2. 2 deep freezers and one reach in cooler not in working order, discussed to remove any unused equipment or repair.
      • Observed severely damaged flooring in front of the 3 compartment sink area.
  • Five Guys Burgers and Fries, 570 Celebrate Virginia #153 Fredericksburg, VA 22405
    • Date inspected: Dec. 19
    • Total violations: 2
      • The concentration of the sanitizer in the 3-comp sink read 500 ppm at 84 degrees F. Water was added to the sanitizer bringing the concentration down to 400 ppm.
      • Observed the ceiling vents and ceiling tiles throughout the kitchen in need of cleaning.
  • Wendy's Store #891, 2301-A Plank Rd Fredericksburg, VA 22401
    • Date inspected: Dec. 19
    • Total violations: 7
      • Observed a food employee change a trash bag, once they finished they fail to wash their hands. EHS discussed violation with the person in charge.
      • Observed a food employee attempt to wash their hands at the 3 compartment sink. EHS discussed with the person in charge to retrain food employees on were to wash their hands.
      • Observed a container of raw burger patties stored uncovered in the walk in cooler. During the inspection the person in charge covered the container.
      • 0ppm was observed at the 3 compartment sink. During the inspection a food employee changed out the sanitizer and 300ppm was recorded.
      • Observed plastic spatulas stored in a container with the food contact surface facing upwards. Discussed with the person in charge, they relocated the spatulas to the warewashing area.
      • Observed racks/shelving holding clean equipment above the 3 compartment sink requires cleaning.
      • Observed floors drains, floors of the dry storage and floors of the walk in freezer require cleaning.
  • King's Pizza & Italian Restaurant, 7983 Kings Highway King George, VA 22485
    • Date inspected: Dec. 20
    • Total violations: 4
      • Observed 2 uncovered coffee mugs on the prep line. Drinks were covered during inspection.
      • Observed a gallon container and a whisk stored in the hand sink near the cook's line. Items were removed from the sink.
      • Observed chicken breasts and ground beef being thawed in the 3 compartment sink without running water. Water was turned on during inspection.
      • The interior on the deep fryer cabinet and the grey cart near the cooktop are in need of cleaning.
  • Mattaponi Springs Golf Club, 22490 Penola Rd Ruther Glen, VA 22546
    • Date inspected: Dec. 20
    • Total violations: 8
      • Raw hamburger that is cooked to order is offered on the menu without having at least one employee with supervisory and management responsibility and authority who is a certified food protection manager to direct and control food preparation and service.
      • The person in charge could not provide verification that employees have been trained on the employee health policy.
      • Observed accumulation of debris on the food contact surfaces of three tongs and two whisks that were stored with the clean dishes on a rack above the three compartment sink. Items were relocated to be cleaned and sanitized.
      • Observed the chemical dishmachine that is used to clean and sanitize dishes measured at 0 PPM. Person in charge will use the 3 comportment sink to wash and sanitize the dishes until the unit is evaluated.
      • Per discussion with the person in charge, dishes and utensils that are stored at room temperature and come in contact with time/temperature control for safety food are washed once a day at closing time. Items must be washed and sanitized at least once every four hours or when contamination occurs. Discussed cleaning frequency with the person in charge. Items were relocated to be washed and sanitized.
      • Raw shell eggs and cooked to order hamburger are offered on the menu without a consumer advisory.
      • The person in charge could not provide a copy of the water sampling report, the report is held off site and will be emailed.
      • Observed the ceiling tiles in the male and female toilet rooms are not smooth and easily cleanable and are absorbent. Observed heavily soiled ceiling tile in the female toilet room.
  • Bee Happy Nutrition, 2142 Richmond Hwy. #104 Stafford, VA 22554
    • Date inspected: Dec. 20
    • Total violations: 1
      • Observed open carton of liquid eggs in mini reach in cooler not dated for disposition.
  • Vinny's Italian Grill & Pizzeria, 7835 Dolleys Ct King George, VA 22485
    • Date inspected: Dec. 20
    • Total violations: 1
      • Observed employee pick up ready-to-eat garlic bread with their bare hands and then wrap it if foil for an order. Instructed employee to discard the bread, wash their hands, don gloves, then handle the garlic bread.
  • The Olive Garden Italian Restaurant #1542, 2811 Plank Road Fredericksburg, VA 22401
    • Date inspected: Dec. 21
    • Total violations: 17
      • Absence of active managerial control observed during the inspection. The person in charge is not monitoring the following: 1. Proper glove usage 2. Cooling and cooling equipment 3. HACCP plan for cook-chill operations are being monitored and completed as approved.
      • Person in charge could not provide documentation employees have been trained on the employee health policy. EHS provided the person in charge with an employee health policy document.
      • Person in charge could not provide procedures for cleaning up vomiting or diarrheal events. EHS provided a sample to the person in charge.
      • Observed a food employee with gloves on handling a box, without changing gloves and washing their hands the employee started peeling eggplants. During the inspection the employee discarded the soiled gloves, washed their hands and donned new gloves.
      • Observed the following Non TCS food contact surfaces Soiled: 1.Interior of ice machines in kitchen 2.Soda nozzle sprayer in bar area 3.Can opener
      • Observed the three compartments sink in the bar area in use, when sanitizer was tested no PPM were measured for quat sanitizer. Person in drained sanitizer from the sink.
      • Observed the following food contact surfaces soiled: 1.Knives inside knife rack 2. Stainless steel pans soiled on the food contact surface stored on the clean dish rack. All items were removed during the inspection to be cleaned.
      • Observed the food establishment is not monitoring and documenting the temperatures of TCS foods prepared in a cook-chill ROP process per the HACCP plan and completing the logs as required to ensure adequate cooling is achieved from the following dates 12/2/23 &12/7/23. Observed ROP bag of Zuppa Toscana soup without date the food was made or a label to identify the food.
      • Observed the salad reach-in cooler unplugged at 72 degrees. The salads inside the reach-in cooler were at 70 degrees. Through discussion with the person in charge salads were made around 10:00 am the day of the inspection. The person in charge relocated the salads to the walk-in cooler to cool them to the proper temperature.
      • Observed inside numerous reach-in coolers no thermometers are inside measure the air temperature.
      • Observed at a prep station numerous utensils in use being stored inside a stainless-steel pan of water at 100 degrees. Person in charge removed the pan of utensils. EHS discussed with the person in charge leaving utensils in foods cold holding at 41 degrees or below.
      • Observed several stainless-steel pans and plastic trays stacked wet on the clean dish rack. All items were removed during the inspection to be air dried.
      • Observed numerous cutting broads heavily scored.
      • Observed the following nonfood contact surfaces in need of cleaning: 1. Interior of cooling equipment 2. Interior of fryer cabinets 3. Dish cart by ware washing area 2. Lids stored soiled on the nonfood contact surfaces(corrected) (Person in charge removed lids to be cleaned) 3. Containers and trays stored with tape like labels left on.
      • Observed sheets pans have encrusted residue build up on the non-food contact surface.
      • Observed the following plumbing in poor repair: 1. Under the prep sink the water pipe is not underneath the air gap. 2. No faucet handles on the mop sink.
      • Observed walls and ceilings throughout the facility are in need of cleaning.
