Fredericksburg Health Inspections: 21 Violations In 1 Kitchen

The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Aug. 23 and Aug. 29.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Orofino Restaurant, 1006 Caroline Street Fredericksburg, VA 22401
    • Date inspected: Aug. 23
    • Total violations: 8
      • Observed the can opener soiled. Person in charge removed it for cleaning.
      • Observed a small stainless-steel container of meat sauce in the prep unit on cookline held at improper temperature. (77 degrees F). The meat sauce was prepared the day prior and was left in the unit over-night. Person in charge voluntarily discarded the item.
      • Observed food employee without a beard restraint.
      • Observed many stainless steel and plastic containers wet while stacked after being sanitized.
      • Observed food employee with gloved hands using cellular device prior to handling food.
      • Observed unsealed wood in the three-compartment sink on the second floor and dump sink in the server station. Also observed rusted shelves in the walk-in cooler.
      • The ceiling tiles and vents are in need of cleaning throughout the kitchen.
      • Observed the following in poor repair: 1) loose ceiling tiles in the kitchen. 2) damaged employee restroom door.
  • Mangoes, 1075 Gateway Blvd. Fredericksburg, VA 22401
    • Date inspected: Aug. 23
    • Total violations: 1
      • Through discussion with the person in charge the owner allegedly owns another food establishment in the area and is preparing raw chicken and transporting it to the food establishment. PIC was unable to communicate if the food is transported in a cooler with ice and the temperature of the items upon arrival. The food establishments are under different business entities and are not an approved food source unless inspection for wholesaling is obtained by Virginia Department of Agriculture and Consumer Services (VDACS). Two containers and one gallon plastic food grade bag of jerk chicken were observed in the homestyle refrigerator. The items were not date marked.
  • Denny's #9575, 10360 Patriot Hwy Fredericksburg, VA 22408
    • Date inspected: Aug. 23
    • Total violations: 15
      • The person in charge could not provide written procedures for clean-up of vomiting and diarrheal events. Sample procedures provided during the inspection.
      • Observed an employee set up a sanitizer bucket and then proceed to touch lids at the hot holding station without washing their hands. Instructed employee to wash their hands.
      • Observed a food employee lather hands under running water, hands were not effectively cleaned due to soap being flushed down the drain. Discussed correct handwashing procedures and the employee washed their hands.
      • Observed interior of two microwave ovens at the cook/prep line were soiled. The person in charge reported they were not used during breakfast service.
      • Through discussion with the operator the soda nozzles are soaked in soda water and sanitized, absence of a wash step. Instructed the operator to wash and sanitize soda nozzles.
      • Observed pancake batter, which is prepared with pasteurized liquid eggs, cold holding on top of prep cooler at the cook line at 45 degrees F. Observed the top of the container stored above the cold well line. The person in charge reported the pancake batter was pulled from the walk-in cooler at 8:00 a.m. Instructed the operator to cool to 41 degrees F, sell or discard by 12:00 p.m.
      • Observed five containers of macaroni and cheese which had a date sticker to discard the product on 6/25/2023. The person in charge reported the wrong date sticker was created and placed a new date sticker on the product.
      • Observed commercially cooked pot roast stored in sealed bags in a reduced oxygen environment (air removed from the packaging) with a 48 hour discard date and time of 8/23/2023 at 1:18 a.m. The facility is properly labeling the product for 48 hours. During the inspection the pot roast was discarded.
      • Observed absence of adequate hair restraints by food employees.
      • Observed the front handsink by the cook line, vegetable prep sink and stainless-steel equipment by the hot wells are not sealed to the adjacent walls/equipment.
      • Observed accumulation of grime and debris on nonfood contact surfaces throughout the facility including shelving, interior and exterior of refrigeration equipment, cooking equipment including trap and suppression lines at the hood, and prewash sprayer. Recommend a deep and thorough cleaning.
      • Observed the following: 1- New convection oven which is connected to the water line in absence of a backflow preventor. The facility installed the equipment without contacting the health department. 2- Hose bibb vacuum breaker installed to a hose with spray nozzle, which had water flowing during the inspection, at the mop sink. Informed the operator a dual check valve with atmospheric vent is required in lieu of a hose bibb vacuum breaker (not designed for continuous pressure). The water was turned off during the inspection. Provided backflow handout during the inspection.
      • Observed floors, walls and plumbing in need of cleaning.
      • Observed the following in poor repair: Gaps in ceiling tiles around conduit cables, ceiling tiles not tight fitting to the frames, peeling/damaged ceiling tiles, cracked floor tiles by cookline and floor tiles in need of grouting in the back of the house.
      • Observed accumulation of debris on hood vents in the food establishment.
  • China Garden, 7028 Salem Fields Blvd Fredericksburg, VA 22407
    • Date inspected: Aug. 23
    • Total violations: 1
      • Observed in the walk-in cooler in cooler raw beef next to zucchini squash and raw chicken next to precooked chicken. During the inspection foods were relocated.
  • ERI/Gladys H. Oberle School, 404 Willis St Fredericksburg, VA 22401
    • Date inspected: Aug. 24
    • Total violations: 3
      • Observed multiple glove use violations, in particular using gloves to open refrigerator and cabinets doors and then going back to food preparation. EHS had a through discussed about glove usage with the person in charge.
      • Observed single service plastic platter tops and punch bowl not inverted in dry storage area.
      • Observed that the multi-quaternary ammonium test strips were expired. The person in charge advised they will get new test strips.
  • Sealston Elementary School, 11048 Fletchers Chapel Road Sealston, VA 22485
    • Date inspected: Aug. 24
    • Total violations: 1
      • Observed the top interior of ice machine soiled. The ice machine was cleaned during the inspection.
  • Subway (Lee's Hill), 10635 Spotsylvania Avenue Fredericksburg, VA 22408
    • Date inspected: Aug. 24
    • Total violations: 7
      • Through discussion with the person in charge they could not provide the cleaning procedure or sanitizer that is used for the deli slicer. EHS discussed with the person in charge wash, rinsing and sanitizing procedure.
      • Facility could not provide a certified food protection manager.
      • The person in charge could not verify documentation of employee health training. EHS provided employee health policy documents during the inspection.
      • Facility could not provide procedures for responding to vomiting and diarrheal events. EHS provided a sample cleanup procedure during the inspection.
      • Observed a food employee touching money then put gloves on to prepare food without washing their hands before putting gloves on. EHS instructed the food employee to remove their gloves and wash their hand before putting new gloves on.
      • Observed no paper towels at the hand sink by the three compartment sink and women's restroom hand sink. During the inspection the person in charge placed paper towels at the hand sinks.
      • Observed the deli slicer soiled, through discussion with the person in charge the deli slicer was last used the day prior to the inspection.
  • Dunkin Donuts, 10245 Patriot Highway Fredericksburg, VA 22407
    • Date inspected: Aug. 25
    • Total violations: 1
      • Observed a food employee preparing food without wearing a hair restraint. The employee put a hair restraint on during the inspection.
  • Riverside Center, 95 Riverside Parkway Fredericksburg, VA 22406
    • Date inspected: Aug. 25
    • Total violations: 6
      • Observed pink build-up on interior plastic lip of the ice machine in the catering hallway. Due to significant build-up and large amount of ice near the plastic lip, instructed the operator to discard the ice and clean and sanitize the ice machine.
      • Observed three spray bottles which contained Fabuloso cleaner. Two spray bottles were not labeled and the third was labeled as Windex. During the inspection the operator labeled the spray bottles to correctly identify the toxic material.
      • Observed three hot shot pest control devices in the food establishment. Two were in the warewashing area and the third near the food preparation area. Pest control devices were discarded during the inspection.
      • Observed the following in poor repair: Ice build-up under the fan inside the bar walk in cooler and walk in freezer, blown out shielded lights under the hood, rust build-up on shelving in dry storage and refrigeration. Instructed the person in charge to relocate packaged food in the walk-in refrigeration units until serviced by a technician.
      • Observed the following in poor repair: Cracked wall under the prewash spray in the dishwasher area, seal coving tile to the wall under the dishwasher, gaps in ceiling tiles around conduit cables, blown out ceiling.
      • Observed the following areas in need of cleaning: Drain and floor near the kitchen ice machine, floors behind the cookline and caulking at the dishwasher area.
  • Brother's Pizza, 3605 Plank Road Fredericksburg, VA 22407
    • Date inspected: Aug. 25
    • Total violations: 2
      • Facility could not provide a certified food protection manager.
      • Observed four flying insects throughout the kitchen area. EHS advised the person in charge to contact the pest control company the facility uses to further assist the issue.
  • HOMEMADE, 7019 Harrison Rd 109 Fredericksburg, VA 22407
    • Date inspected: Aug. 29
    • Total violations: 1
      • Observed the concrete floor around the 3-compartment sink is no longer sealed. Discussed applying a smooth and easily cleanable sealant to the floor. Ensure flooring in the food preparation and areas subject to moisture are smooth, easily cleanable and nonabsorbent.
  • Tokyo Express, 5700 Salem Run Blvd #106 Fredericksburg, VA 22407
    • Date inspected: Aug. 29
    • Total violations: 8
      • The person in charge was unable to provide proof the employees had received training concerning Employee Health. EHS provided employee health documents to the person in charge during the inspection.
      • Facility could not provided a procedure for responding to vomiting and diarrheal events. EHS provided a sample during the inspection.
      • Observed in the walk-in cooler raw beef over green peppers. Foods were relocated during the inspection.
      • Observed two prep cooling units not working and through discussion with the person in charge are not being used.
      • Observed cardboard taped to the vent above the hood vents. Cardboard was removed during the inspection.
      • Observed wire shelving throughout the facility and in the walk-in cooler rusted.
      • Observed the following non food contact surfaces in need of cleaning : 1. Exterior of cooking equipment 2. Interior of prep cooler 3.Floor fan
      • Observed employees personal foods throughout the facility including the walk-in cooler and prep cooler. EHS discussed with the person in charge having a designated area food employees foods.
  • Roma's Pizza, 114 W Broaddus Ave Bowling Green, VA 22427
    • Date inspected: Aug. 29
    • Total violations: 10
      • Person in charge was unable to provide a current certified food protection manager certificate. Person in charge said one employee is already scheduled to attend the next class on 9-11-23.
      • Observed a food employee handle raw meat, place it on the flat top grill, take off his gloves, put on new gloves, and then go back to handle cooking utensils for the flat top. The person in charge had employee take off the gloves, wash their hands, and then put on new gloves.
      • Observed the dishwashing machine dispensing no chlorine to sanitize items, the test strips were reading 0ppm after three runs of the dish machine. Person in charge will call today for service, and use the 3 compartment sink to manually sanitize items, until machine is fixed.
      • Observed the menu had a consumer advisory at the bottom of the menu, but the items offered to be undercooked were not marked with an asterisk(*).
      • Observed the back door near the ware washing area does not self close.
      • Observed single use articles, aluminum to go containers and aluminum baking pans, being stored with the food contact surface facing up. The person in charge inverted all items.
      • Observed the inside door panels of the dish machine heavily soiled.
      • Observed the following heavily soiled: 1. the outside of the dish machine; 2. the seals on the walk in cooler and walk in freezer; 3. the outside of the outdoor grease receptacle.
      • Observed the walk in cooler lighting was reading 1.0-4.6 foot candles using a light meter.
      • Observed A/C vents in the toilet rooms and in the kitchen areas soiled.
  • Bowling Green Elementary School, 17502 New Baltimore Road Milford, VA 22514
    • Date inspected: Aug. 29
    • Total violations: 1
      • Observed the seals of equipment throughout the facility soiled.
  • Feast Buffet, 2388 Plank Rd Fredericksburg, VA 22401
    • Date inspected: Aug. 29
    • Total violations: 21
      • Observed lack of managerial control based on the following: 1.) monitoring food temperatures. 2) not being able to provide documentation of an employee health policy 3.) risk of food contamination while in storage, etc.
      • Observed a food employee wash their hands in a food prep sink.
      • Observed a bowl and rag in the basin of the handwashing sink next to the three-compartment sink. Items were removed.
      • Observed mussels over pineapple and hard-shell eggs over RTE foods. Items were relocated.
      • Observed the ice machine and can opener soiled.
      • The dish machine is not dispensing sanitizer after running three cycles with a ppm reading of 0. The dish machine was fixed during the inspection.
      • Observed the following foods on the buffet line at improper temperature: chicken wings (96 degrees F), egg rolls (111 degrees F), and plantains (109 degrees F). The foods were put out at 11:00 am and were on the buffet line for 1 and half hour. All foods were reheated to 165 degrees F and above.
      • Observed no proper date marking system throughout the kitchen.
      • Observed many fish in the prep sink thawing in still water. The water was turned on.
      • The screen door of the back door has out openings.
      • Observed several fly strips hanging from the ceiling throughout the kitchen, fly swatters on top of foil pans in the dry storage area, and fly insect device over cups in the server station.
      • Observed several boxes and trays of raw chicken on the floor in the walk-in freezer. Person in charge relocated the chicken.
      • Observed food employees without hair and beard restraints.
      • Observed two knives in between the prep table and two door reach in cooler. Items were removed for cleaning.
      • Observed several plastic containers wet while stacked after being sanitized.
      • Observed rust on shelving racks throughout the kitchen.
      • Observed several cutting boards heavily stained.
      • Observed several encrusted debri on the outer surface of sheet pans.
      • Observed the following non-food contact surfaces in need of cleaning: 1) equipment throughout the kitchen. 2) shelving throughout the kitchen. 3) up-splash of the soda machine. 4) rice cookers on the cookline.
      • The floors, ceilings, and walls throughout the kitchen are in need of cleaning.
      • Observed employee belongings (keys, drink, jacket) throughout the kitchen.
