Fredericksburg Health Inspections: 26 Violations At 1 Location
The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.
FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Feb. 22 and Feb. 28.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."
Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Papa Johns Pizza #285, 3940 Plank Rd Suite W Fredericksburg, VA 22407
- Date inspected: Feb. 22
- Total violations: 3
- Observed the following in need of cleaning: 1. floor drain under 3 compartment sink 2. corners of floors and under equipment 3. dusty AC vents and also side fan vents of the pizza oven
- Observed food employees working without a beard restraint.
- Person in charge was unable to provide a written plan/procedures or a spill clean up kit for bodily fluids. Per discussion with the person in charge the facility does have a system and plan in place for bodily fluid clean ups just not in writing. EHS provided a sample plan and instructed PIC to adapt to the facility needs or to write a plan out.
- The Pub, 4187 Plank Road (Chancellor Center) Fredericksburg, VA 22407
- Date inspected: Feb. 22
- Total violations: 4
- Observed containers of chicken stored with the shrimp in the walk in cooler. The person in charge rearranged the products.
- Observed single service boxes stored on the floor. The person in charge placed the boxes on a crate.
- Observed unnecessary equipment like the outdoor grill and the large pizza oven in the kitchen.
- The following physical facilities are in need of cleaning: light shields and the floors behind the ice machine.
- Panera Bread #805, 1760 Carl D. Silver Parkway Fredericksburg, VA 22401
- Date inspected: Feb. 22
- Total violations: 12
- Person in charge was unable to provide a verifiable way of the facility having an employee health policy in place. During the inspection EHS provided a copy of a policy. Person in charge ensured employees read and signed the policy before the inspection was over.
- Observed a food employee scrub their soapy hands for less than 5 seconds. EHS explained proper handwashing procedures to the food employee, they rewashed their hands.
- Observed the interior surfaces of all of th microwave ovens in the facility were soiled.
- Per discussion with the person in charge, the steam wand of the espresso machine currently does not undergo the wash rinse and sanitize step every 4 hours. The steam wand gets sanitize in between uses only.
- Observed food employees working and preparing foods while wearing wrist jewelry.
- Observed rusted wire shelving throughout the facility.
- Observed cutting boards that are heavily stained and scored.
- Observed numerous sheet trays/baking pans with heavy build up encrusted build up of grime and other debris.
- The following non food contact surfaces were observed heavily soiled with food debris and grime: 1. surface areas underneath microwave ovens 2. rolling carts 3. inside and upper parts of the prep cooling units in the drive thru area 4. wire shelving throughout the facility 5. up splash of the coffee brewers
- Observed all of the kitchen hand sinks were soiled.
- Observed the following in need of cleaning: 1. floors throughout the facility, especially under equipment 2. floors of the walk in cooler and walk in freezer 3. floor drains throughout 4. walls behind all prep cooling stations and behind beverage stations.
- Observed the following facilities in disrepair: 1. loose and damaged ceiling tiles in the kitchen area 2. a light out in the baker's cooler.

- Date inspected: Feb. 22
- Total violations: 7
- The person in charge could not provide written procedures for clean-up of vomiting and diarrheal incidents. Provided sample written procedures during routine inspection conducted 2/25/2022 and during today's inspection. Informed the person in charge if the facility uses a disinfectant other than Chlorine bleach, which is included in the sample procedures, the facility will need to develop their own written procedures.
- Through discussion with food employees the espresso machine steamer wands, which are in contact with TCS beverages, are removed for cleaning and sanitizing once daily. The wands are being sanitized between uses. Informed the person in charge equipment in contact with TCS foods are required to be cleaned and sanitized at least once every four hours. Food staff agreed to comply.
- Observed stainless steel half wall behind the Starbucks dump and front hand sink are not sealed to the adjacent walls.
- Observed accumulation of debris on the following nonfood contact surfaces: Interior base of refrigeration and freezer equipment throughout and shelving at the Starbucks station.
- Observed the floors, ceiling tiles and ceiling light covers throughout the facility in need of cleaning.
- Observed the following in poor repair: 1- Numerous ceiling tiles are not sealed to the frames, observed gaps and 2- gap in ceiling tiles which tubing lines are run through the ceiling.
- Hood vents above the TurboChef oven in need of cleaning, significant dust accumulation observed.
- Crown Catering Service, 6376 Patriot Highway 204 Spotsylvania Courthouse, VA 22553
- Date inspected: Feb. 22
- Total violations: 3
- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- The interior surfaces of the microwaves were observed soiled. Both ovens were cleaned during inspection.
- The following non-food contact surfaces were observed soiled: 1-the exterior of the deep fryers, 2-the cooktops, 3-the spice containers, 4-the desk fan, 5-the food warmers that are being used for equipment storage, 5-the base of the grinder.
- Jay's Downtown Sports Lounge, 409 William Street Fredericksburg, VA 22401
- Date inspected: Feb. 24
- Total violations: 5
- Observed 5 cutting boards that were heavily soiled and a soiled china cap hanging from the clean pot rack. Items were taken to the dish machine to be cleaned.
- There was no temperature measuring device located in the cold holding drawers, the cook's prep unit and thunder counter freezer.
- The door gaskets on the walk in cooler door, the cold holding drawers, the cook's prep unit and the under counter reach in freezer were observed torn.
- The sides of the deep fryer cabinets and the under counter reach in freezer were of served soiled.
- The wall behind the dish machine and the floors under and behind the equipment on the cook's line were observed to be heavily soiled.
- Holiday Inn Express & Suites, 6415 Dan Bell Lane Thornburg, VA 22565
- Date inspected: Feb. 24
- Total violations: 2
- Consumer self-dispensing bulk food bin containing cereal is not properly labeled.
- The walls in the kitchen were observed soiled.

- The person in charge could not provide written documentation pertaining to bodily fluid cleaning.
- Observed that the can opener (corrected) and the pizza oven require cleaning.
- Observed that the sprayer at the 3-compartment sink and the can opener holder requires cleaning.
- Observed that the walls over the pizza prep unit, computer wiring over the pizza prep unit, and the air vent covers near the pizza oven are in need of detailed cleaning.
- Freddy's Frozen Custard, 28 S Gateway Drive Fredericksburg, VA 22406
- Date inspected: Feb. 27
- Total violations: 4
- Observed a food employee touch her face then applied gloves to prepare food without first washing their hands. Observation was discussed with person in charge (PIC)- employee was asked to wash their hands.
- Observed raw flounder fish (45'F-178'F) not cooked to a sufficient temperature to eliminate pathogenic bacteria. Food employee intentionally cooked two flounder fish fillets, instead of one as they were stuck together, therefore not all parts of the fish were properly cooked. According to the cooking instructions on the box, the fish have to be cooked from a frozen state to 350'F for 3-4 minutes. According to the PIC Frozen fish get cooked for three minutes and thirty seconds. EHS instructed PIC to individually wrap the raw fish, PIC will purchase food grade bags. For the meantime, PIC will ensure the fish is being separated from one another prior to storing in the reach in freezer by the fryers. Corrected during the inspection.
- Facility was using an antimicrobial fruit and vegetable treatment without having proper testing strips. Employees mix the chemical with water manually as the dispensing unit is not working properly. EHS instructed PIC to cease from using the product until the unit is repaired and the proper testing strips are obtained. PIC placed a do not use sign on the unit.
- Observed several spray bottles of all purpose cleaners stored near exposed single service articles. Items were removed.
- McDonald's of Stafford #10738, 2862 Richmond Highway Stafford, VA 22554
- Date inspected: Feb. 27
- Total violations: 2
- Observed food employees without proper hair restraints working around exposed food and equipment. During the inspection food employees placed hair nets on.
- Observed the food establishment installed a new frozen beverage machine, which is connected to the water line, without a backflow prevention device installed. Observed a water pressure reducing valve installed on the water line in lieu of a backflow preventer. During the inspection an adequate backflow prevention device was installed.
- Chick-fil-A Garrisonville, 25 Dunn Drive Stafford, VA 22554
- Date inspected: Feb. 27
- Total violations: 2
- Observed a food employee handling two containers of waffle fries from the interior of the container- their hands were touching the waffle fries- observation was discussed with the person in charge. Corrected during the inspection.
- Observed eight containers of pre-made salads (42-43'F) stored in the cold holding unit (41'F) near the fried chicken hot holding station- salads were prepared six hours prior to temperature check- salads were discarded. PIC contacted a technician to evaluate the unit- the unit will not be used until it is evaluated.
- Miso Asian Grill & Sushi Bar, 1305 Emancipation Highway Fredericksburg, VA 22401
- Date inspected: Feb. 27
- Total violations: 13
- Observed an employees child in the kitchen. EHS explained to the person in charge only food employees of the establishment are allowed in the kitchen area. The employee and child left the facility.
- Person in charge was unable to provide a signed employee health policy. During the inspection EHS provided a copy. PIC had employees read and sign the policy.
- Person in charge was unable to provide written procedures for clean up of vomiting or diarrheal events. During the inspection EHS provided a copy of a VDH written procedures, advised the PIC to adapt procedures to the facility needs.
- Observed a food employee touch their face mask with a gloved hand attempt to proceed to prepare foods. Person in charge directed food employee to remove gloves and wash hands.
- Observed the interior lip making direct contact with ice need cleaning.
- Observed single service containers being used as scoops stored inside the food bins (rice, spices). During the inspection person in charge removed the containers.
- Observed stainless steel and plastic containers that were stored stacked while still wet.
- Observed a heavy ice build up inside the surfaces of the deep freezers.
- The following non food contact surfaces require cleaning: 1. inside surfaces of reach in freezers 2. metal wire shelving throughout the facility (in walk in cooler, holding clean equipment, holding dried foods) 3. inside surfaces of the cooling equipment
- Observed a direct connection in plumbing at the 3 compartment sink. EHS provided a diagram to the person in charge on the necessary air gap needed as a backflow prevention n the plumbing of the 3 compartment sink set up.
- Observed the following in poor repair: 1. 4 blown out lightbulbs in the kitchen area and 1 blown out light bulb in the hood vent area 2. damaged/cracked floor tiles 3. loose/peeling ceiling tiles
- Observed floors and walls throughout the kitchen that require cleaning.
- Observed employees, coats, hats, backpack, cell phones and keys stored above or on the same shelf as foods. During the inspection employees belongings were relocated.
- Grounds Bistro, 25 Apricot Street Stafford, VA 22554
- Date inspected: Feb. 27
- Total violations: 17
- Observed a food employee wash their hands in the kitchen hand sink for less than 20 seconds. Instructed the food employee on correct handwashing and the employee washed their hands.
- Observed a food employee enter the kitchen without washing her hands and begin to prepare foods. Instructed the food employee to wash her hands and discussed issue with the operator. The food employee did not wash her hands.
- Observed absence of handwashing sign in the right-side unisex toilet room. Provided a handwashing sign during the inspection.
- Observed 3-4 soiled knives on the magnetic knife rack and a soiled stainless steel food storage container on the warewashing shelving. The equipment was removed for cleaning and sanitizing.
- Observed the panini press plates heaving encrusted to debris. Discussed deep cleaning of the plates or replacing the unit if the equipment is no longer cleanable.
- Observed spring mix inadequately cooled within 4 hours. Observed spring mix at 45 and 47 degrees F stored in an overfilled covered plastic container in the 1 door upright cooler #2. A food employee reported the spring mix was pulled from the downstairs refrigeration at 10:30 a.m. and placed in refrigeration in the kitchen. Temperatures were observed at 2:00 p.m. The spring mix was placed into smaller portions and relocated to the 2 door upright cooler located downstairs. the spring mix was observed at 45 degrees F at 2:35 p.m. and was voluntarily discarded for safety. Recommended preventing overfilling of containers to facilitate rapid cooling. Discussed using time as a public health control during the inspection.
- Observed absence of proper disclosure for hamburgers which may be cooked to order upon a customer's request. During the inspection a food employee placed as asterisk next to hamburgers on the menus which directs customers to the reminder statement of the consumer advisory.
- Observed food employees without proper hair and beard restraints.
- Observed food employees wearing wrist jewelry while preparing food in the kitchen.
- Observed absence of measurable Quaternary Ammonium sanitizer in a bucket which was being used to store multiple wiping cloths between uses. During the inspection the sanitizing solution was discarded.
- Observed three scratched and scored cutting boards.
- The person in charge could not provide an irreversible registering temperature indicator to monitor sanitization of the high temperature dishwasher.
- Observed accumulation of debris on nonfood contact surfaces throughout the kitchen.
- Absence of covered receptacle in the unisex toilet rooms. Observed waste receptacle with an opening for disposal of all waste.
- Observed significant accumulation of leaves and other brush debris in the dumpster enclosure.
- Observed open ceiling above the bar area which beverages are prepared and warewashing of barware is conducted. The ceiling is not smooth or easily cleanable. The person in charge reported quotes for repair are being obtained to correct the ceiling surfaces to come into compliance.
- Observed the floors, walls and ceiling tiles in the kitchen are in need of cleaning.
- Carrabba's Italian Grill, 1951 Carl D. Silver Pkwy Fredericksburg, VA 22401
- Date inspected: Feb. 28
- Total violations: 15
- Person in charge was unable to provide EHS with signed copies or any other verifiable of the facility having an employee health policy. During the inspection EHS provided a copy, also food employees were able to answer questions regarding foodborne illnesses and symptoms associated.
- Observed the men and womens bathroom had no hand washing signs available. During the inspection EHS provided hand washing signs.
- Observed a large plastic container of broccoli stored in the walk in cooler uncovered. During the inspection the person in charge had an employee cover the container of broccoli.
- Observed the upper interior surfaces of the ice machine soiled with black mold like growth.
- Observed a food employee grilling a chicken breast, EHS took an temperature of the grilled chicken breast (per food employee chicken was fully cooked) VDH thermometer registered an internal temperature of 156-160 degrees F. EHS advised food employee to continue cooking the chicken breast. EHS re-took a temperature-165 degrees F. EHS advised food employee and person in charge to ensure employees use a calibrated food thermometer to ensure foods are being fully cooked.
- Observed a container of marinara sauce hot holding at 127-133 degrees F. Marinara sauce had been placed in the hot holding unit 2.5 hours prior to the inspection. Person in charge discarded the marinara sauce for safety.
- In the dry storage room observed 3 cans of kidney beans and 1 can of chick peas heavily dented.
- Observed numerous stainless steel containers that were stored stacked while still wet.'
- Observed rolls of paper towels and hand towels used in the main bathrooms stored on a shelf in the employees bathroom.
- Observed a food employee wearing gloves handling soiled dishes proceeded to handle clean dishes with the same pair of gloves.
- Observed several cutting boards heavily stained and no longer seem cleanable.
- Observed the following equipment in poor repair: 1. handles missing from under counter reach in units 2. ice build up at the back of the walk in freezer and around the door frame of the walk in freezer 3. ice build up inside the ice cream deep freezer.
- Observed the following non food contact surfaces in need of cleaning: 1. inside the doors of the fryer cabinets 2. non food contact surfaces of the mixers 3. wire/metal shelving throughout the facility 4. handles of equipment 5. wheels and sides of cooking equipment 6. cart used to transport soiled dishes
- Observed the following facilities in need of repair: 1. ceiling tiles held with blue tape by the back door 2. 1 missing ceiling tile in the kitchen 3. grout missing throughout the kitchen floors 4. cracked floor tiles throughout the kitchen area
- Observed the following facilities that require cleaning: 1. floors underneath equipment 2. floor drains throughout the facility 3. walls in the dishroom and walls behind food prep areas 4. stained and soiled ceiling tiles and light shields throughout the kitchen area 5. floors of the walk in cooler and walk in freezer 6. floors of the mop area
- Don Eladio's Cantina, 4585 James Madison Pkwy King George, VA 22485
- Date inspected: Feb. 28
- Total violations: 2
- Observed a knife and other utensils being stored in sanitizer. Items were removed from the sanitizer and allowed to air-dry.
- There is no covered receptacle for sanitary napkins in the toilet room used by females.
- Ledo Pizza, 4599 James Madison Pkwy King George, VA 22485
- Date inspected: Feb. 28
- Total violations: 6
- In use open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils. Observed an uncovered drink on the pizza prep unit. Drink was discarded during inspection.
- The interior of the microwave oven was observed soiled. Oven was cleaned during inspection.
- The cutting boards along the prep units were observed heavily stained.
- The cutting board(s) along both prep units are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
- The following non-food contact surfaces were observed soiled: 1-the exterior surfaces of the flattop grill, 2-the deep fryers, 3-the table and shelves under the flattop grill, 4-the garbage cans throughout the kitchen, 5-the outer surfaces of the of the prep units, 6-the small fan above the grill prep unit, 7-the floor fan, 8-the cvent in the walk in cooler, 9-the exterior and rear of the combi-oven.
- The following physical facility items were observed soiled with grime and debris: 1-the walls throughout the kitchen, especially behind the dish machine and the dough mixing area, 2-the floors under the equipment on the cook's line, 3-the floors in the walk in cooler and freezer, 4-the ceiling tiles around the HVAC vents.
- Charter Senior Living of Fredericksburg, 20 Heartfields Lane Fredericksburg, VA 22405
- Date inspected: Feb. 28
- Total violations: 26
- Observed absence of active managerial control during the inspection. Through discussion and direct observation the person in charge is not monitoring handwashing of food employees, food temperature of time/temperature control for safety food that is being hot held is not being monitored, sanitizer solution used for food contact surface is not being monitored to ensure it is at the proper concentration- person in charge was testing the lactic acid sanitizer with the quaternary ammonium testing strips. PIC is not monitoring temperatures of time/temperature control for safety foods upon delivery. Discussed requirements during the inspection.
- No certified food protection manager available.
- The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
- Person in charge (PIC) was chewing gum in the kitchen during the inspection. EHS instructed PIC to discard the gum.
- Observed a food employee handling soiled equipment then clean equipment without washing their hands. Employee was asked to wash their hands.
- Improper handwashing procedures observed. Employee washed hands for less than 10 seconds. Employee was instructed to re-wash their hands.
- Observed several tongs, ladles, spatulas, and knives soiled to the sight and touch. Items were moved to be washed and sanitized.
- Observed interior plastic lip of the ice machine soiled.
- Observed a container of mechanically diced commercially processed ready to eat chicken (107'F), a container of peas (122'F), a container of pureed commercially processed ready to eat chicken (108'F)- items were placed in the unit for thirty minutes- items were reheated in the microwave to above 165'F.
- Observed an unlabeled spray bottle that contained toxic stored in the kitchen near the prep sink. Bottle was labeled during the inspection.
- Observed an insect control device located above three large food containers.
- Observed four employees working in the kitchen- two employees were observed conducting food preparation process wearing wrist jewelry.
- Observed a food employee with artificial nails handling exposed food while not wearing gloves.
- Observed several wet wiping cloths stored in a bucket (no solution) under the counter and on the counter. Items were removed.
- Observed cooking utensils stored where contamination may occur: 1) two whisks stored on hooks with the hooks being in contact with the food contact surface of the items and 2) three knives stored between the wall and the knives holder, 3) tongs used in the hot holding was stored in the serving container while touching the food. Items were moved to the warewashing area to be washed.
- Observed several food plate covers stored while wet on the counter. Items were removed to be washed, sanitized and adequately dried.
- Silverware were observed stored unprotected on the dining room tables.
- Observed the following: 1) a food employee setting up tables in the dining room area- employee had a cart with a rack of silverware that was stored with the lip contact surface facing different directions. 2) exposed single service service articles exposed in the storage room located in the lower level. Observations were discussed with the person in charge.
- Observed two melted spatulas and three broken knives. Items were discarded.
- Observed the following: 1) several rusted shelves, 2) the grill was being used without the hood filters being placed properly. The person in charge placed the filters properly.
- Lactic acid sanitizer was being used without a proper testing kit.
- Accumulation of debris was observed on the following nonfood contact surfaces: 1) reach in coolers, 2) top of knife holder, 3) upsplash of the coffee brewer.
- Observed a hose attached to a Y connector at the mop sink without a a back flow prevention method.
- The floors, walls, and ceiling (to include ceiling vents and light shields) are in need of cleaning- re-caulk the drainboard of the dishwasher and 3 compartment sink.
- Observed the following in poor repair: 1) Gap in ceiling tiles near the cooking area, 2) ceiling tiles not secured to the frame in the warewashing area, 3) broken floor tiles by the warewashing area, 4) missing coving in the mop sink.
- Observed the ceiling in the mop sink not smooth or easily cleanable.
- Family Food Palace (VA G72 138), 916 Churchill Dr Fredericksburg, VA 22407
- Date inspected: Feb. 28
- Total violations: 3
- The person in charge could not provide written documentation regarding employee health training.
- Observed that the inside of the reach in refrigerator, top of the fryers, inside of the fryer cabinet, and drop in freezer require cleaning.
- Observed that the floors, walls, and ceiling in the mobile unit to include the floor mat is in need of detailed cleaning.
